Job summary

Enthusiastic and highly motivated Practice Nurse required to join our shared nurse team across East and Park Parade Surgeries in Harrogate.

Main duties of the job

You will work alongside existing experienced practice nurses, a trainee nurse associate and healthcare assistants for peer support.

The candidate will, ideally, be able to demonstrate a good understanding of all routine Practice Nurse duties including complex dressings, immunisations of both children and adults and long term conditions management.

The role is part time for 1-2 days per week (8-15 hours) with the possibility of an early start (at 7.30am) or a late finish (8pm). Pay is depending on skills and experience. We offer NHS pension membership and BUPA health plan. If you have any questions please emailsarah.wilkinson22@nhs.net

Practice Nurse

ORGANISATION STRUCTURE

Administrative responsibility to the Practice Manager

JOB PURPOSE

The practice nurse should assess the care needs of patients and implement and evaluate programmes of care for patients in conjunction with nursing colleagues and GPs according to practice policy, protocols and best nursing practice.

MAIN RESPONSIBILITIES AND ACTIVITIES

Main nursing responsibilities

1 Assess the nursing needs of patients, develop a nursing plan and provide care/treatment as appropriate taking into account patient/carer choice and according to practice policy, protocols and nursing practice

2 Review / evaluate the effectiveness of nursing care provided and where appropriate modify nursing care plans as required

3 Support patients to adopt health promotion strategies that encourage patients to live healthily, and apply principles of self-care

4 Deliver opportunistic health promotion using opportunities such as new-patient medicals

5 Deal with general nursing care of the patient including:

a. Wound care including complex diagnosis, management and treatment of leg ulcers

b. Management of skin disorders

c. Immunisations & vaccinations (Baby & Adult)

6 Assist in the provision of care to patients with chronic disease including hypertension; diabetes, asthma, COPD and coronary heart disease

7 Assist with womens health including cervical cytology; advising on sexual health and family planning

8 Undertake routine screening eg frailty, memory loss and refer appropriately

9 Where appropriate, teach and co-ordinate the work of primary health care assistants and other members of the team who may have responsibility for implementing specific aspects of care
