Job summary
We are looking for a forward thinking, conscientious and compassionate
Nurse to join our nursing team. You will
be registered with NMC and may be newly qualified or already have experience of
working within a GP Practice or PCN setting.
There is the opportunity to develop within the role and learn new
skills.
Main duties of the job
Scope
& main responsibilities:
a.
To assess the needs of patients ensuring the
provision of healthcare is appropriate, incorporating evidence-based practice.
To develop, implement and embed health promotion and wellbeing programmes
b.
To implement and evaluate individual treatment
plans for chronic disease patients that promote health and wellbeing
c.
To identify, manage and support patients at
risk of developing long-term conditions, preventing adverse effects to
patients health
d.
As required, to provide routine nursing care
to patients in accordance with clinical based evidence, NICE and the NSF
e.
Work within own competence and professional
code of conduct as regulated by the NMC
f.
Produce accurate, contemporaneous and complete
records of patient consultation, consistent with legislation, policies and
procedures.
g.
Prioritise, organise and manage own workload
in a manner that maintains and promotes quality.
h.
To be able to perform all practice nurse
responsibilities such as childhood immunisations and cytology
18-24 hours per week
£29000-£35000 pro rata depending on experience

About us
We are a two site practice based in the heart of Burgess Hill,
West Sussex. We provide GMS to a
registered list of 13,600 patients. We
have a team of 5 Partners, 6 associate GPs, a paramedic practitioner, clinical
pharmacist, care coordinators, social prescribers, an experienced nurse team
including 3 nurses and 3 HCAs and a very experienced and compassionate
reception team. We have an experienced
senior management team, led by our Practice Manager and we use EMIS clinical
system, AccuRx and eConsults.
We are a member practice of the Burgess Hill Villages Primary Care
Network, along with three other local practices. We strive to provide the highest level of
clinically safe and sustainable service to our patients whilst enabling our
practice team to flourish within a supportive and family-friendly

occasion, a requirement to carry out other tasks. This will be dependent upon
factors such as workload and staffing levels:

To be involved in the implementation and evaluation of individual

Learn effective triage skills, utilising all members of the
clinical team.

As required, to provide routine nursing care to patients in

Learn how to diagnose and manage acute and chronic conditions

Learn how to review patient medications to enhance compliance and
effectively manage repeat prescribing
G.
Be able to provide guidance to patients in the use of prescribed
and over-the-counter medications regarding side effects and dosages
H.
To understand the provision of travel medicine services

To learn how to request pathology services and process pathology
results as required
J.
To be able to understand the provision of chronic disease clinics,
delivering patient care as necessary, referring patients to
secondary/specialist care as required

To maintain accurate clinical records in conjunction with extant
