Main duties of the job
The purpose of the Practice Nurse is to deliver safe, high quality primary care nursing services to the practice population. Working within the NMC Code of Conduct and professional boundaries, the post holder will deliver primary health care interventions and care.
The post holder will support service development, the safe delivery of policy and procedures, and demonstrate effective professional nurse standards, advocacy and performance

Lakeside Healthcare is a large GP partnership that provides NHS care for around 170,000 patients across Cambridgeshire, Peterborough, Northamptonshire and Lincolnshire. We are made of eight GP practices across 14 sites that are part of their local communities.We work together as practices to provide resilience, sharing learning and professional back-office support services.

- Evidence of continued professional development
- Mentorship course or equivalent
Desirable
- Primary Care Nursing Qualification
- Degree in healthcare/Nursing
- Accredited Diploma level training in chronic disease management
- Clinical supervision training and experience
Experience
Essential
- Recent experience as a Registered Nurse in a primary care setting
- Involvement in implementing and using protocols and clinical guidelines
- Experience of managing/dealing with the public/patients
- Experience of audit and/or research
- Proactive in improving and influencing the patient care pathway/journey
- Demonstrates the ability to manage clinical risk and has an understanding of healthcare governance systems
- Experience of using clinical systems and Microsoft office
Desirable
- Management of long-term conditions
- Participation in quality initiatives such as clinical benchmarking
- Experience of SystmOne
