The PCN Community Nurse role is focused on delivering safe, high-quality primary care nursing services to the St Neots Primary Care Network (PCN) population.

The post holder will work proactively across practice and community settings, supporting service development while providing responsive, patient-centred care.

The role requires close integration with the wider multidisciplinary team to assess, manage, and support patients with ongoing medical and nursing needs.

St Neots PCN has a great Nursing/AHP team with a wide range of skills, we have a good support network and offer different training opportunities.

Please note the salary offered is the part time equivalent. This is a role offering 25 hours per week.

Main duties of the job

The purpose of the PCN Community Nurse is to deliver safe, high quality primary care nursing services to the practices population. Working within the NMC Code of Conduct and professional boundaries, the post holder will deliver primary health care interventions and care. The post holder will support service development, the safe delivery of policy and procedures, and demonstrate effective professional nurse standards, advocacy and performance.
