We have an exciting opportunity for an Advanced Nurse Practitioner to join our team within St Neots PCN, where you can make a
positive contribution to patient care and peer support and supervision.
This role
provides the opportunity to work across PCNs sites and to become involved in
Population Health management across this PCN.
As an Advanced Nurse Practitioner you will be acting within
your professional boundaries, providing care for patients from initial history
taking, clinical assessment, diagnosis, treatment and evaluation of their care
with the support of the PCN Clinical Lead as required. You will demonstrate
safe, clinical decision-making and expert care for patients within the Primary
Care Network (PCN), whilst working autonomously and taking full accountability
for patient encounters. You will work within your professional boundaries as
part of the Multi-Disciplinary team (MDT) across the PCN, supporting the
delivery of the Primary Care Directed Enhanced Service to meet the needs of
patients.
Main duties of the job
- Assess and triage patients, including same day triage, and as appropriate provide definitive treatment (including prescribing medications following policy, patient group directives, NICE (national) and local clinical guidelines and local care pathways) or make necessary referrals to other members of the primary care team.
- Manage undifferentiated undiagnosed condition and identify red flags and underlying serious pathology and take appropriate action
- Use complex decision making to inform the diagnosis, investigation, complete management of episodes of care within a broad scope of practice.
- Actively take a personalised care approach and population centred care approach to enable shared decision making with the presenting person.
- Complete the relevant training in order to provide multi-professional clinical practice and CPD supervision to other roles within primary care, for example first contact practitioners and the personalised care roles.
