Job summary

At Lakeside Healthcare as part of the PCN expansion, we are looking to employee two part-time (20 hours and 25 Hours per week) Safeguarding Care Co-ordinators. The successful candidate will form part of the busy safeguarding team based at Forestgate Surgery.

The Safeguarding team covers all the GP surgeries within the Lakeside Healthcare Group and the 4 PCNs across the county and the ability to travel between sites is essential.

We require staff who share a passion for ensuring adults and children within our geographical area, are supported and given the opportunity to achieve in life.

The ideal candidates will be highly motivated, flexible and able to work competently under pressure within a busy working environment.

We require you to have excellent communication, planning, organisational and prioritisation skills and the ability to work well as a member of an increasingly busy and demanding team. Previous experience of working in an organisation delivering care to vulnerable children and adults will be of value.

Main duties of the job

  • Through liaison, research and dialogue identify and collate relevant health information to add to multi-disciplinary risk assessment discussions and Multi-Disciplinary Team Meetings (MDTs).
  • Under supervision of the Head of Safeguarding, provide timely and accurate case conference reports and health safeguarding reports as requested, as part of the Multi-agency Safeguarding processes across all areas within the Primary Care Network.

Job Summary: To carry out the role and responsibilities of a Safeguarding Care Co-ordinator for the Safeguarding Team on behalf of the Primary Care Network Practices.

Job Responsibilities: The post holder will work as a member of the Safeguarding Team to ensure a high-quality safeguarding service is delivered to all PCN GP Surgeries within Rockingham Forest PCN, South Peterborough PCN, St Neots PCN and Four Counties PCN. This will include providing advice, support and relevant information to a multitude of professionals, enabling the Safeguarding Team to assess the most appropriate plan of action for each adult and child case reviewed.

Main Duties:

Through liaison, research and dialogue identify and collate relevant health information to add to multi-disciplinary risk assessment discussions and Multi-Disciplinary Team Meetings (MDTs).

Under supervision of the Head of Safeguarding, provide timely and accurate case conference reports and health safeguarding reports as requested, as part of the Multi-agency Safeguarding processes across all areas within the Primary Care Network.

Attend key meetings as required, such as the practice Multi- Disciplinary Team Meetings (MDTs).

Collate and record accurate minutes of any child and adult safeguarding meetings attended and ensure GP actions from these meetings are processed in a timely manner.

Provide support and guidance to all Primary Care Network staff with support from the Head of Safeguarding.

Under the guidance of the Head of Safeguarding, proactively review case files for individual patients in relation to levels 2-4 of local Thresholds and Pathways procedure.

Ensure patient records are reviewed and maintained by the team, complying and adhering to any GDPR, Subject Access requests, safeguarding report requests whilst ensuring patient confidentiality in line with the seven golden rules of information sharing.

Seek to obtain written and or verbal consent for release of any GP information unless to do so would be detrimental to the child or adult or cause further risk. In line with the Primary Care Network Practice polices.

Respond to requests for health information in a timely and professional manor.

Contribute to the collection of data for safeguarding metrics that accurately reflects the safeguarding team activity.

Participate in audit activity as required within the safeguarding team.

Manage and maintain the safeguarding information database, to ensure cases are reviewed in line with agreed timescales.

Raise any safeguarding concerns directly to the Head of Safeguarding/ Safeguarding Nurse or the Safeguarding practice GP for adult and children.

Contribute to the delivery of safeguarding information and training for all practices within the Primary Care Network.

Your normal place of work will be Lakeside Healthcare Forest Gate in Corby or within the associated Primary Care Network and its practices, or such other place as the Group may reasonably determine ability to travel within these practices will be required at various times during your working week.

Personal Development:

Receive supervision from the Head of Safeguarding on a regular planned basis in recognition to the potential impact of working in safeguarding on self and others. Seek support and help where necessary.

Attend bi-monthly 1-1 meetings with the Head of Safeguarding to quality assess work and develop clear actions of individual work and progress within the team.

Participate in yearly appraisals.

Work with the Head of Safeguarding to develop an individualised education plan to enhance knowledge and progression within the safeguarding team. This will include completion of the Personalised Care Institute Curriculum as part of the Primary Care Network education programme.

Attend a variety of venues within the county to ensure updated knowledge of local safeguarding working procedures/ practice requirements.

Attend countywide training sessions as required or as identified within the individualised educational plan.

Other Tasks:

Duties may be varied from time to time under the direction of the Primary Care Network, Practice Director/ Practice Manager, dependent on current and evolving practice workload and staffing levels.
