This role is part of our new Collaborative Care Team which aims to promote a multidisciplinary approach to care where primary, community and voluntary sectors work together to provide tailored support to help people live well and independently at home for longer. Giving them more say about their care and the support they receive and offering more support to those who look after family members, partners or friends.

Main duties of the job
You will be assessing patients with undifferentiated medical conditions, using clinical assessment skills, and organise appropriate treatment plans in conjunction with the patient. You will liaise with GPs and other professionals, make onward referrals and organise diagnostic tests as appropriate. This will involve the provision of evidence based care in the assessment, diagnosis and treatment of those with minor illnesses, long term conditions and complex conditions. This may also require the provision of palliative care.
Essential Criteria
1 Clinical Assessment Qualification
2 Independent Prescriber
3 Current experience of working in an independent and semi-autonomous role, diagnosing and treating patients.
4 Demonstrates the ability to manage clinical risk and an understanding of health care governance systems.
5 Ability to priorities, delegate and organize own workload, meeting and achieving deadlines.

About the Practice/Department/Team
Lakeside Healthcare Corby operates over 3 sites and are rated as GOOD with CQC. We have 30 people in our nursing team across various roles all supported by our Lead Nurse.
We have a strong team ethos and believe that including those delivering the service in decisions around how the service works are key to the best patient outcomes so actively engage with all parts of the surgery to achieve this.
We have created a supportive environment through clinical supervision, team and quality meetings and an open and honest working relationship across our team.
We continue to invest in the training and development of our nursing team and recognise the importance of personal as well as organisational development.
As a practice we like to undertake projects to improve both patient and staff experience and have recently signed up to the Green Impact Toolkit for Practices. We also enjoy supporting local and national charitable events and regular social events.
