Main duties of the job
The post holder will work within their clinical competence as part of a pharmacy team to provide expertise in clinical medicines management and performing medication reconciliation and annual medication review of patients.
The post holder will provide quality improvement and clinical audit and manage some aspects of the Quality and Outcomes Framework. This role is pivotal to improving the quality of care and operational efficiencies so requires motivation and passion to deliver an excellent service within general practice.

Essential Criteria
· Master’s degree in pharmacy (MPharm) (or equivalent)
· Registration as a Pharmacist with the General Pharmaceutical Council
· Evidence of CPD
· Independent prescriber (or nearing completion) with experience of managing chronic disease
· Postgraduate diploma in Clinical Pharmacy and/or Completion of the CPPE Pathway (or nearing completion)
Attitude and approach are just as important as qualifications and we are looking an individual who is confident to work autonomously and proactively to deliver targets. You should also be dedicated and compassionate with excellent communication and time management skills and have a thirst and enthusiasm to make a difference in primary care.
