Job summary

This is a Beverley PCN role for working within our organisation.

Initially based at our main site in Beverley but may involve work, when needed, at any of our other 4 sites including one at Cottingham, although we would try to keep this to a minimum.

We are a keen training practice with years 3-5 HYMS and Registrars and have a full compliment of clinicians including ANP, Prescribing Pharmacist, HCA, GPA's, Phlebotomists and 13 partners. We have a full support team including Care Navigators and Managers and participate fully in the PCN day to day workings.

High QOF achievers and low turnover of staff.

Looking to employ an ANP or ACP through Beverley PCN and happy to consider a minimum of 26 hours up to full time for the right candidate.

Main duties of the job

Clinical Nursing Practice

  • Work as part of a multi-disciplinary team to deliver safe and high quality care.
  • Carries out clinical nursing practice at an advanced level using expert clinical skills, critical judgements & knowledge to deliver appropriate and safe care.

Delivery of care in line with NSF, NICE guidelines and evidence-based care

  • Triage and treat patients, performing specialist assessment of their needs and make referrals as necessary to other members of the primary and secondary health care teams as appropriate.

Co-ordinate the planning and delivery of care ensuring patients and carers/relatives are involved.

  • Prescribing and reviewing medication that is appropriate to patient needs and in accordance with latest evidence-based practice, national and practice protocols and within the scope of qualified practice.
  • Diagnose and manage acute and chronic conditions, including drug and non-drug based treatment methods into a management plan.

Request diagnostic investigations and interpret within agreed protocols e.g. x-ray, blood tests, scans, etc.

  • Assess, diagnose and treat patients in practice premises or their own home who require medical attention and/or refer to appropriate agency.
  • Refers patients to other members of the MD practice Team, secondary care organisations and other organisations as necessary.

About us

This role will operate under Beverley PCN which cover approx £50k patients across Beverley and Cottingham with 4 partners practices, including ourselves.

We have 5 sites, 4 in Beverley and 1 in Cottingham, with 13 partners.

We are a HYMS training practice with high numbers of students, mainly at our Molescroft site. We also help operate the Registrar training with 2 currently.

We are a high QOF achieving practice with a dedicated team running Long Term Conditions and also have MSK FCP working at our sites, in house medicines management team and prescribing Pharmacist. We have 3 ANP's, one does clinic and acute visits, the other two do clinics each working day with a visit if needed.
