Job summary
The Cambridge Northern Villages PCN are seeking two enthusiastic and forward thinking Pharmacy Technicians to join our team of
Clinical Pharmacists to deliver quality of service to their patients.
The role will include delivering effective medicine
management and medicines optimisation, improving patient care and being part of
a multidisciplinary team, whilst working within the strategic objectives of the
PCN.
Primary care experience would be an advantage, however it
is not essential. You should hold a BTEC/NVQ Level 3 qualification in
pharmaceutical sciences and be registered with the General Pharmaceutical
Council. (If you have not enrolled, you should be willing to enrol on the CPPE
Primary Care Pharmacy Education Pathway.
This is an evolving role with a huge scope for
development and improvements as we expand our PCN.

Main duties of the job
To
liaise with patients, clinical and non-clinical staff, hospital and community
pharmacists, care homes and patients carers to improve the pharmaceutical care
of patients.
To
support the PCN Clinical Pharmacists as the PCN point of contact for all
medicines related matter; to maximise support to all clinical and non-clinical
practice staff to improve patient care in relation to all aspects of medicines
and their impact.
To
support in the management of the repeat prescribing process, helping to manage
repeat prescription requests, resolving queries where possible and producing
prescriptions for clinical review and signature.
To
ensure patients have appropriate monitoring tests in place when required such
as: regular blood tests, INRs and blood pressure.
To
undertake medicines reconciliation of hospital letters and discharge summaries
in line with local protocols and within competency levels.
To
promote and implement electronic repeat dispensing in appropriate patients; to
promote online ordering of prescriptions.
To
work alongside the PCN clinical pharmacists to deliver certain aspects of the
PCN Direct Enhanced Services (DES)
To
undertake medication audits in priority areas, such as QOF, NICE guidance, DSQS
and other agreed targets areas, feeding your findings back to the network teams
and implementing actions as appropriate.

1 First Contact Physiotherapist, 1 Social Prescriber and 1 PCN Manager

Whilst this role is being considered as a Hub based model, working from one of our PCN locations (as above) applicants must be aware that travel to all locations may be required from time to time to support our Clinical Pharmacists.

The post holder
will work within their professional boundaries for each GP practice within

of safe, effective and efficient systems for repeat prescribing, medicines
optimisation, reducing medicines waste and maximising patient outcomes.
To

promote the Clinical Commissioning Group (CCG) Medicines Management initiatives
and annual projects
To
undertake medication audits in priority areas, such as QOF, NICE guidance, DSQS
and other agreed targets areas, feeding your findings back to the network teams
and implementing actions as appropriate.
Provide
independent information to patients with regard to medicines and prescribing
changes, initiating further support from other healthcare professionals where
appropriate.
To
take a proactive approach in the safe handling of prescriptions.
To
support the training and development of prescription clerks (if required)
To
undertake all mandatory and induction training.
Supervision:
The post holder
will line managed by the PCN Manager, however, all clinical supervision will be
managed by the Senior Clinical Pharmacist, supported by the lead GP from the
practice whose patients the post holder is dealing with.
