Cambridge Northern Villages PCN
(Primary Care Network) is looking to recruit a Social Prescriber to join our
PCN team.

We need a talented and empathetic
individual with excellent communication skills to be part of a growing team.
The successful person will work across multiple surgeries, supporting people
who have long-term conditions, are lonely or isolated, or who have complex
social needs which affect their wellbeing.
This is a non-clinical role, but
one that supports patients to address and tackle issues that are having an
adverse effect on their mental and physical health such as housing, debt or
social isolation.
Social prescribing empowers
people to take control of their health and wellbeing through the support of
non-medical link worker. The focus is holistic approach, connecting people to
community and voluntary sector groups and statutory services for practical
emotional support. Social prescribing can help to strengthen community and
personal resilience, reducing health inequalities by addressing wider barriers
and increasing peoples active involvement in local communities.
We are looking for someone with
relevant qualification, skills and resilience to work with GPs and practice
staff, helping people by addressing their needs in a non-medical way.
Main duties of the job
Cambridge Northern Villages PCN (Primary
Care Network) is looking to recruit a Social Prescriber to join our PCN team.
Social prescribing Link worker can help
to strengthen communities and personal resilience, reducing health inequalities
by addressing the wider determinant of health such as debt, poor housing and
physical inactivity, by increasing peoples active involvement with their local
communities.
Social prescribing is particularly
effective for people living with long term conditions (including support for
mental health), for people who are lonely or isolated, or have complex social
needs which may affect their wellbeing.
This is a non-clinical role, but
one that supports patients to address and tackle issues that are having an
adverse effect on their mental and physical health such as housing, debt or
social isolation.

referrals from a wide range of agencies (including by not exhaustive)

Job responsibilities
To
be a friendly source of information about wellbeing and prevention approaches.
To
help people identify the wider issues that impact on their health and
well-being such as debt, poor housing, being unemployed, loneliness and caring
responsibilities.
To
help people to maintain or regain independence through living skills,
adaptations, enablement approaches and simple safeguards.
To
work with individuals to co-produce a simple personalised support plan.
Where
people may be eligible for a personal health budge, to help them to explore
this options as a way of providing funding, personalised support to be
independent, including helping people to gain skills for meaningful employment,
where appropriate.
To
forge strong links with local VCSE organisations, community and neighbourhood
level groups, utilising their networks and building on whats already available.
To
work with commissioners and local partners to identify unmet needs within the
community and gaps in community provision.
To
support local partners and commissioners to develop new groups and services
where needed, through small grants for community groups, micro-commissioning
and development support.
To
support in the delivery of enhanced services and other services requirements on
behalf of the PCN.
To
deliver training, mentoring and guidance to other clinicians and staff on SPLW
matters.
To
produce Social Prescribing newsletters or bulletins on a quarterly basis.
To participate in the management of patient complaints when requested to
do so and participate in the identification of any necessary learning brought
about through incidents and near-miss events.
To manage a caseload of potentially complex patients and to provide
advice for the GP management on the more complex patients.
To
review the latest guidance ensuring the practice conforms to NICE, CQC etc.
Support
the delivery of QOF, incentive schemes, QIPP, IFF and other quality or cost
effectiveness initiatives.
To
actively signpost patients to the correct healthcare professional.
To
provide targeted support and proactive reviews for the vulnerable, complex
patients and those at risk of re-admission to secondary care.
To
work with your line manager to access regular supervision to enable you to deal
effectively with the difficult issues that people present.
To
contribute to public health campaigns (eg: Flu & Covid clinics) though
advice and direct care.
Making effective use of training to update knowledge and skills.
Using appropriate infection control procedures, maintaining work areas
in a tidy and safe way and free from hazards.
Reporting potential risks identified.
Acting in a way that recognizes the importance of peoples rights,
interpreting them in a way that is consistent with procedures and policies, and
current legislation.
Respecting the privacy, dignity, needs and beliefs of patients, carers
and colleagues.
Behaving in a manner which is welcoming to and of the individual, is
non-judgmental and respects their circumstances, feelings priorities and
rights.
Participate in any training programme implemented by the network as part
