Job summary
An exciting opportunity has arisen for a Paramedic to join the Picton Primary Care Network as part of our growing Network Team. Our dynamic network team already consists of 3 Clinical Pharmacists, 2 Physician Associates, 2 Physiotherapists and 2 clinicians in our Mental Health Team.
We are keen to support the successful candidates in completing their prescribing qualification as soon as possible as well as other qualifications that would benefit both the candidate and the network.
They will also receive additional experience and support by shadowing clinics of one of our excellent Physician Associates, who has a Paramedic background.
Several practices within the network are well established training Practices and have a strong track record working with clinicians moving over to primary care.
Main duties of the job
The post holder will be responsible for carrying out consultations to patients at the practices, as well as visiting patients in their place of residence, to assess and treat patients presenting with minor or acute illness. This will include urgent and same day requests for treatment and chronic disease management, providing care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment, and evaluation of care.
The post holder will need to demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills. There will be the need to prioritise and triage the needs of patients accordingly making any necessary referrals for investigations in the appropriate manner.
The candidate must own their own vehicle as home visits will be an expectation.
About us
Our Network of practices operates across the Picton Neighbourhood which are all a ten minute drive from each other and includes Edge Hill Health Centre, Sefton Park Medical Centre, Dunstan Village Group Practice, Earle Road Medical Centre and Picton Green Family Practice.
Our combined patient population is approximately 35,000. Our network works very closely with our community team and other local Healthcare providers. The network adopts new methods of working via an extended clinical workforce.

Undertake the collection of pathological
specimens including intravenous blood samples, swabs, and other samples within
their scope of practice, and within line of local and national guidance.

Support the delivery of anticipatory care plans
and lead certain services (e.g. monitoring blood pressure and diabetes risk of
elderly patients).
·
They will provide an alternative model to urgent
and same day GP home visit for the network.
·
Communicate at all levels across organisations
ensuring that an effective, person-centred service is delivered.
·
Communicate proactively and effectively with all
colleagues across the multi-disciplinary team, attending and contributing to
meetings as required.
·
Maintain accurate and contemporaneous health
records appropriate to the consultation, ensuring accurate completion of all
necessary documentation associated with patient health care and registration
with the practice.

Communicate effectively with patients, and where
appropriate family members and their carers, where applicable, complex and
sensitive information regarding their physical health needs, results, findings,
and treatment choices.
