Learning Disabilities and Severe Mental Health Support
TaSR PCN has a sizable cohort of patients who require enhanced
support in the booking and attendance of their Learning Disability and Severe
Mental Illness reviews. Part of this
role is to actively encourage engage patient with appointment booking,
reminders and follow up calls. It is
important to build a connection with patients and patient representatives to
ensure positive outcomes and good patient experiences.
Patients with LD and SMI diagnoses require additional
support which may be in the form of alternative communication. It will be important for the right candidate
to be able to clearly identify patients who require other methods of
communication (letter/phone call/text/relay to family member). They should also be able to clearly
communicate what is involved in the patient's annual review and how this will
benefit them.
The role should also be a key contact throughout the key
for needs relating to LD or SMI (not urgent/acute needs).
Population Health Management
The Primary Care Network wish to support the Practices by
engaging in preventive disease management (NHS Health Checks). NHS Health checks are for eligible patients
without a current long term medical conditions.
Checks can identify whether patients' are at a higher risk of heart
disease, diabetes, kidney disease and stroke.
The role supports the identification of patients via
reporting through to the end consultation of patients in Practice. You will work closely with the PCN Clinical
Care Coordinator who will be seeing these patients directly.
Social Prescribing Triage
Social Prescribing is a social prescription to gain advice,
support and empower patients to engage with the community and beyond. Social Prescribing is not available to all
patients within the PCN and therefore it is important to triage and signpost
those who are eligible to local services.
Triaging also acts as a useful tool to ensure our in house team see
patients who cannot gain support elsewhere.
Other Duties
The role may also provide additional capacity to support
the booking and administration of any national vaccination programmes the PCN
may take part in to include the Covid Vaccination Program.
Team rota management on behalf of the PCN Manager.
Care home reporting and other generalised reports.
The role will also be work alongside other PCN clinicians
and AHP's to support the administration of the local care homes assigned to the
PCN as well as being responsible for new patient registrations and online sign
up.
Job
Summary
The aims of the role are to:
1. Provide
additional support to PCN member Practices in the delivery of the NHS Health
Check by providing non-clinical admin support (bookings).
2. Support
the delivery of the PCN Directed Enhanced Service (DES) and Impact and
Investment Fund (IIF) and Quality Outcomes Framework (QoF)
The role will work closely with existing GP practices multi-disciplinary
team structures ensuring the patient experience remains central to every stage
of their journey.
The post holder will work with the multi-disciplinary team
to ensure that all requests for support or patients needs are assessed and put
into action wherever appropriate.
Principal
Responsibilities
1. Provide
support and encourage attendance for LD and SMI Reviews.
2. Provide
additional capacity for Practices by supporting the uptake of NHS Health checks
in addition to other clinical duties as directed by the Practice teams.
3. Provide
a single point of contact to staff to navigate to appropriate resource for the
Weight Management ES.
4. Ensure
patients who are referred to the Social Prescribing service are appropriately
triaged within 3 working days and directed as per the agreed pathway.
Deliverables
Assist
with the delivery of seasonal vaccinations (if competent to do so).
Run and
manage rotas.
Ensure the
designated caseload is professionally managed and that the care and treatment
provided is in line with Practice policies and procedures.
Conduct
duties in a way that reduces impact on Practice activities and increases
overall capacity.
Produce
reports of LD, SMI, NHS Health Check uptake and engagement as well as other
reports at the request of the PCN team.
Identify
patients and produce reports on those who would benefit from early
intervention.
Ensure
a coordinated approach to service deliver at all PCN sites to promote good
working relationships.
Ensure
Statutory and Mandatory training is kept up to date as well as any additional
CPD at the request of the PCN Practices.
Your role will involve but not be limited to you performing
the above.
Monitoring
and Reporting
Assist
in the production NHS reports
Provide
quarterly reports to PCN Manager and PCN Board.

in any training programme implemented by the Practice as part of this
employment such training to include:
