Job summary
An exciting opportunity has arisen to join the Wetherby Primary Care Network (PCN) Team as our PCN Operational, Digital and Transformation Manager. This role will form an integral part of the PCN leadership team working alongside the Clinical Director and the PCN board members.
We are looking for someone who will lead on business planning to ensure the smooth operational running of the PCN. The postholder will ensure the network is kept up to date with national and local policies and manage the contractual requirements of the Network Directed
Enhanced Service.
The post holder will build strong
relationships with PCN member practices and will support the planning and
delivery of improvements of services. The role will require experience in
quality and service improvement with expertise in transformational change and a
good understanding of general practice systems and processes and in particular
the use of digital tools for improved ways of working
Main duties of the job

To provide project management for the implementation of PCN initiatives
and transformation
To maintain a high calibre of understanding with national polices
relating to the PCN DES. Support local
PCN developments, in line with local (CCG and GP Confed) and national (NHSE)

To work closely with our Clinical Director and Practice Managers from our member practices to understand current service models, identify challenges, agree priorities for improvement and support these to be delivered in line with the current guidelines and the PCN DES
To be the key point of contact and representative at ICB and other stakeholder meetings
To support the day-to-day operational management to the PCN workforce
To track and provide data on the IIF targets and other PCN income streams to maximise income for the PCN
To have oversight of PCN budget and Finances to ensure the PCN is maximising and managing funding available
To build strong relationships with GP Practices and allied service partners in
the development projects

Duties And Responsibilities
Business and Service Improvement
- Ensure all relevant PCN national strategies, policies, targets and frameworks are considered and implemented
- Prioritise co-developing mechanisms involving all practices to enable delivery of the PCN DES and high achievement in the Investment and Impact Fund (IIF)
- To provide up to date data measuring PCN achievement in terms of DES and IIF delivery and support and work with practices which may be experiencing difficulties achieving PCN targets
- Support PCN practices to deliver the PCN Capacity and Access Plan and explore opportunities to identify areas for improvement in patient access to Primary Care
- Support the PCN with writing business and development plans as required
- Identify and record risks and issues, developing contingency plans with service and contract leads
- To manage the recruitment, induction and training of new and existing PCN staff contracted via ARRS
- To provide management and support to PCN employed staff and to manage their operational rotas and leave requests as required
- To manage rotas for PCN led Clinics and day to day management for rota changes and staff absences
- Management of complaints
- To support appraisal of PCN staff as appropriate
- Financial management of the PCN, including budgets setting and strategic financial planning. Management in collaboration with the PCN leadership and Leeds GP confederation banking service
- Lead for the development of digital initiatives in supporting practices and staff to optimise use of existing functionality and new technology to deliver benefits for patients and staff and enable improvement
- Use data to support population health management and workforce planning
- Advise on innovative opportunities and support practices to maximise benefits from innovation, through the sharing of learning and best practice
IM&T
- Ensure the PCN is compliant with GDPR and ICO along with other relevant legislation and professional information management standards such as GMC/BMA and LMC guidance
- Manage the PCN IT resources such as laptops, mobile phones and IT equipment
Strategy and Leadership:
Co-development of a PCN strategic vision which will inform the choices we make in terms of clinical delivery. Clarify objectives and priorities for network delivery in line with operational realities, financial parameters and contract requirements
- To deliver on decisions, priorities and business plans coming from the PCN board
- Promote the engagement and co-working of PCN GP practices
- To build robust relationships with the senior managers of the PCN GP practices
- Support and champion the development of good quality locality network plans that deliver relevant change and improvement projects which can be both measured and evaluated
- To be the PCN lead for Health Inequalities; identifying areas of need; project managing identified improvements
- To be the PCN representative at Local Care Partnership (LCP) meetings
- Organise PCN events e.g. Educational/Locality/Network wide events as required

Information and data management
- Take a data driven approach, looking at evidence and information available to support understanding of challenges, identify possible solutions and track the impacts of changes that have been made
- Work in partnership with the ICB to identify relevant data sets and to support standardisation in the use of measures for understanding change and improvement in general practice
- Bring together data from a variety of sources and use information to develop insights into working practices, or support population health management, risk stratification or workforce planning, effectively tailoring content to meet the needs of the audience
- Effectively present and communicate data to practices and PCN staff at all levels to increase understanding of operational challenges
- Interpret data and highlight variation to support open discussion about where variation is and is not warranted
- Help PCN and practice staff understand the limitations of particular data sets and where other information may be available to supplement understanding of issues
