PART TIME ROLE (FT also considered)
An exciting new opportunity has arisen for an Advanced Paramedic Practitioner (AAP) to join our ever-expanding Primary Care Network
team. The PCN is working together with
other AHP's and support staff to delivery a personalised level of care to the
population alongside Practice teams.
We are actively involved in collaborative working and are
looking for an innovative and motivated APP clinician to help us meet the
challenges and opportunities which lay ahead.
Tadcaster and Selby Rural (TaSR) PCN are a well established
3 Practice PCN with approximately 30,000 patients, covering the large
geographical area surrounding Tadcaster, Sherburn-in-Elmet and South Milford.
The right candidate should be caring, dedicated and
compassionate. They will also need and open
and flexible attitude towards working with a variety of patient needs. Good written and verbal communication skills
as well as strong time and case management abilities are needed.
The successful candidate should have a passion and drive for
providing high quality and personalised care.
General Practice experience is desirable but not essential. Prescribing qualification essential.
Main duties of the job
The successful candidate should be an experienced
practitioner who, acting within their professional boundaries, will provide
care to the presenting patient, from taking initial history, clinical
assessment, diagnosis, treatment, and an evaluation of care. They will demonstrate safe clinical decision
making and critical thinking.
The candidate may also act as a clinical leader in the
delivery of services within the PCN or Practice, whilst ensuring the patients
needs are a priority. They will support
staff development to maximise potential and contribute to the development of
local policies and protocols where asked.
As part of the PCN team the APP will play an important role
to proactively identify and work with patients to navigate their healthcare
journey whether this be in a first contact position or longer-term
support.
To support the member Practices and Clinical Director in the
delivery of the PCN Direct Ehnaced Service Specification and Impact and
Investment Fund by working closely with the PCN Manager and PCN Executive
Board.
The candidate would also be expected to assist with the
Covid19 vaccination programme or other national related programmes as directed
by the PCN Manager.
About us
Our PCN team current consists of a range of other AHP and
supporting staff. To include Community
Paramedics, Mental Health Practitioner, Health and Wellbeing Coaches, Social
Prescribers and Care Coordinators.
The PCN works over 3 Practice sites (South Milford Surgery,
Sherburn Group Practice and Tadcaster Medical Centre). The PCN has 1 Clinical Director and is
managed by the PCN Manager.
The postholder will participate in any training programme
implemented by the PCN as part of this employment, such as training to include
participation in annual individual performance review, maintaining a record of
own personal and professional development, learning and performance.
The postholder will support the quality, diversity and

Assess, diagnose, plan,
implement and evaluate treatment/interventions and care for patients presenting
with an undifferentiated diagnosis.
Clinically examine and assess
patient needs from a physiological and psychological perspective, and plan
