An exciting opportunity has arisen for the newly adapted role of Mental Health Care Coordinator within Tadcaster and Selby Rural (TASR) Primary Care Network (Band 4 £21-24K pro-rata).
The role will work across 3 Practices within the PCN (South Milford Surgery, Tadcaster Medical Centre, and Sherburn Group Practice) and has a population of around 28,200 patients spread over a largely rural geographical area. The role will work across 3 Practices within the PCN (South Milford Surgery, Tadcaster Medical Centre, and Sherburn Group Practice) and has a population of around 28,200 patients spread over a largely rural geographical area.
This role would ideally suit someone with a Healthcare Assistant background.
Main duties of the job
The
Practices will be working together to deliver an enhanced and personalised
level of care to the patient population.
As part of the PCN team the Mental Health Care Coordinator will play an
important role to proactively identify and work with patients to navigate their
long-term condition care as well as managing a caseload of active patients who
are due a yearly review. They will also
support patients to understand and manage their conditions alongside other
allied health professions and signposting teams.
The
main part of the role will involve managing a caseload of patients who are
eligible for their mental health long term condition review. The Mental Health
Care Coordinator would be responsible for contacting patients and or relatives
via telephone and booking an appointment with an appropriate clinician and
sending a reminder if required. These patients would then be case tracked and
managed. Other duties may include
actioning reports linked to the Impact and Investment Fund and other PCN dictated projects to include responcibility for Practice Infection Prevention audits and NHS Health Checks for carers.
The candidate would also be expected to assist with the Covid19 Vaccination Programme or other vaccination programs as managed by the PCN. This includes the opportunity for weekday and weekend overtime.

and flexible attitude towards working with challenging patients. The right
candidate required good written and verbal communication skills as well as
having strong time and case management abilities.
Although
the role in main will be administrative there could be future opportunities to
progress onto completing some patient facing non-clinical elements of the
reviews. This would be subject to the
right candidate, the appropriate training and development pathways. This will not be the main part of the role.
