Job summary

We are initially looking for a General Practice Nurse on an 18-month contract to cover university placement of another member of the nursing team. There is scope for this role to develop into a permanent role. This vacancy is to cover Monday and Tuesday.

Main duties of the job

The General Practice Nurse will be responsible for the implementation and delivery of nursing services at Hope Farm Medical Centre, working as part of the multidisciplinary team, to deliver practice nursing care to our patient population. The General Practice Nurse will be responsible for a number of clinical areas including health promotion, chronic disease management, as well as supporting the medical centre in reviewing and the delivery of clinical policy and procedure. The General Practice Nurse will undertake reviews in line with the Quality and Outcomes Framework (QOF) including diabetes, Asthma and COPD, they will also undertake cytology, vaccinations, and childhood immunisation clinics.

Hope Farm Medical Centre will ensure that the post-holder has access to appropriate clinical supervision and an appropriate named individual in the organisation to provide general advice and support on a day-to-day basis.

The following are the core responsibilities of the General Practice Nurse. There may be, on occasion, a requirement to carry out other tasks. This will be dependent upon factors such as workload and staffing levels:

  • To support the Infection Prevention Control (IPC) lead nurse and to work to the IPC Policy supporting the implementation of audits, training, support, and evidence of compliance to the practice manager
  • To support the production of the IPC annual statement in conjunction with the Infection Prevention Control (IPC) lead nurse
  • To develop, implement and embed an effective care management programme for the frail and housebound patients registered at the practice

  • To assess the needs of patients ensuring the provision of healthcare is appropriate, incorporating evidence-based practice
  • To develop, implement and embed health promotion and wellbeing programmes
  • To implement and evaluate individual treatment plans for chronic disease patients that promote health and wellbeing

  • To identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to patients health
