Are you an experienced Practice Nurse looking for a new and rewarding role? We are recruiting for a 21 hour Practice Nurse who will deliver nursing services collaboratively
with our wider multidisciplinary team to meet the needs of our practice population. If you have experience in Chronic disease management, cervical cytology, family planning, immunisations and leg ulcers then we would like to hear from you.
Main duties of the job
This role provides nursing care to patients including vaccinations, wound care and chronic disease management. Assisting with assessment, planning and delivery of nursing care collaboratively with the medical team. Conducting health assessments, including taking patient histories and performing physical examinations. Administering medications and treatments as prescribed by physicians, whilst maintaining accurate and up to date patient records using our electronic health record system.

Frome Medical Practice functions in a fantastic purpose built surgery with excellent facilities and a large supportive team around the GP. It has out-patients department, mental health services, district nurses, pharmacy, home visits team and health connectors, all on the premises.

Qualifications
Essential
- Registered Nurse with current NMC registration
- Diplomas in the management of chronic diseases or family planning or sexual health or commitment to work towards
- Cervical screening (or willingness to work towards)
Desirable
- Independent Nurse Prescriber (or willingness to work towards)
- NMC's 'Academic Assessors' 'Practice Supervisors' and 'Practice Assessors Accreditations' (or willingness to work towards)
Experience
Essential
- Minimum of one year post qualifying
Desirable
- Experience of primary care
- Chronic disease
- Family planning
- Sexual health
Skills and Knowledge
Essential
- An understanding for the need to adhere to strict confidentiality.
- Ability to work as a team or autonomously
- Ability to use own judgement, resourcefulness, common sense, and local knowledge to respond to the needs of patients

- Competent IT skills
