Job summary

Are you an experienced Nurse Practitioner looking for a new and rewarding role? We are recruiting within our on the day care team. Our on the day teamwork alongside our GPs to provide care for our patients, long term condition and/or acute experience welcomed.Dependent on skills and interest, there is the opportunity to work across both the on the day and complex care teams as a hybrid role.

Main duties of the job

The role would clinically focus on minor illness and on-the-day presentations. We have a mixture of dedicated NP clinics and team working alongside GPs. So successful candidates will demonstrate the ability to work both independently and as part of a team. Learning and continuing development is key with annual appraisal and weekly education opportunities. We value colleagues who have special interests and the enthusiasm to share their knowledge and expertise. We are also offering the opportunity for this to be a standalone or a hybrid role working in our complex care team (see separate job description). The practice is looking for someone who is enthusiastic and keen to develop their skills and are happy to support training to enable all aspects of the role to be fulfilled.

We are a single site practice and Primary Care Network in our own right with a patient population of just under 30,000, in the attractive market town of Frome and surrounding villages. Frome Medical Practice functions in a fantastic purpose built surgery with excellent facilities and a large supportive team around the GP. It has out-patients department, mental health services, district nurses, pharmacy, home visits team and health connectors, all on the premises.

Job description

Job responsibilities

To undertake face to face and some telephone clinical assessment, diagnosis and treatment supporting the practice to meet patient on-the-day and pre-booked demand.

Processing clinical and administrative tasks/results/paperwork including written referrals and electronic referrals such as telederm and discussions with hospital specialties (e.g.Cinapsis)

Ensure that all patient information following consultations, registration, and/or including telephone encounters are fully and recorded accurately on the computer, using EMIS and coded as appropriate.

Optimising the patient journey and teamworking by liaising with GPs to aim to reach a satisfactory assessments and management plans in areas where you are unsure.

Willingness to engage in training and development within the practice including contributing to the training of Nursing colleagues, HCAs and clinical students.

To support the practice in maintaining excellent levels of care including accurate coding to achieve QOF targets and PCN level enhanced services.

Working with the wider clinical team including GPs and Practice Nursing Teams as necessary to flexibly deliver assessment and care to patients on tasks/projects, Annual Reviews and QI projects.

To participate in the formulation of practice protocols in conjunction with NHS England and Somerset CCG.

  • Professional Registration RGN BSc (Hons)
  • Membership of a professional body e.g., RCN.
  • Independent Nurse Prescriber or commitment to work towards on acceptance of the role
  • PACR qualification or commitment to work towards this on acceptance of the role

Desirable

  • Mentoring qualification or equivalent

Experience

Essential

  • Advanced post registration experience in a health care setting
  • Evidence of appropriate continuing professional development
  • Experience of telephone-based triage and consulting.

Desirable

  • Experience working in primary care setting with evidence of appropriate knowledge base
  • Long Term conditions, Sexual Health, Family Planning
  • Experience managing a caseload, complete assessments, plan interventions, implement, and evaluate outcomes according to care plans.
