Job summary
You will
be an experienced registered Advanced Nurse Practitioner who, acting within
their professional boundaries, will provide care for patients registered at the
practice from initial history taking, clinical assessment, diagnosis,
treatment, evaluation of care, prescribing of medication and development of
management plan. The role will involve
care planning and working in the community with the practices patients who may
be housebound, vulnerable adults or residents in care/residential homes.
Main duties of the job
You
will demonstrate safe, clinical decision-making and expert care, including
assessment and diagnostic skills, for patients within the general practice. The
post holder will demonstrate critical thinking in the clinical decision-making
process You will work collaboratively with the general practice team to meet the
needs of patients, support the delivery of policy, procedures, future practice
strategy and provide clinical leadership as required.

Assess
and treat patients presenting with acute illness or for review of chronic
disease conditions.
Diagnose,
plan, implement and evaluate treatment/interventions and care for patients
presenting with an undifferentiated diagnosis
Clinically
examine and assess patient needs from a physiological and psychological
perspective, and plan clinical care accordingly
Diagnose
and manage both acute and chronic conditions for adults and children,
integrating both drug- and non-drug-based treatment methods into a management
plan
Prescribe
and review medication for therapeutic effectiveness, appropriate to patient needs
and in accordance with evidence-based practice and national, local and practice
protocols and formularies
Work
with patients and their carers in order to support compliance with and
adherence to prescribed treatments
Provide
a telephone triage service, assessing patients health needs and management
plans. Implement and evaluate clinical interventions to meet those needs.

Essential
- Registered first level nurse with recent relevant L7 qualifications and current registration
- Relevant nursing/health degree
- Mentor/teaching qualification
- Extended / independent nurse prescribing qualification
- Up to date life support training
Desirable

- Accredited LBC Cytology Trainer
- Teaching certificate
- Diabetes diploma / COPD diploma / Asthma Diploma
