Job summary
Southview Surgery is further expanding our clinical team and are
looking for a professional, motivated, experienced clinical Pharmacist
practitioner to join our team.
Main duties of the job
The successful candidate will utilise their patient facing
clinical practice and independent prescribing qualification to work alongside
our established clinical team to support our medicines management agenda manage
patients with chronic disease and develop their skills in seeing and treating
patients as part of our multidisciplinary team.
Duties
will include the ability to evaluate treatment/interventions and provide care
for patients with undifferentiated and undiagnosed problems using clinical
assessment skills in the Primary Care setting, working as part of our
multidisciplinary duty team as well as assisting with long term condition
clinics and medicines management for the practice.

Job responsibilities
This is an additional clinical pharmacist post within the
practice and we are seeking an experienced Clinical Pharmacist Practitioner
with sufficient skills, enthusiasm and commitment to fulfil and develop the
role within our practice team.
You
will work autonomously with an extended role (within own competency) providing
a combination of on the day appointments, alongside medicines management,
chronic disease management and pharmaceutical advice to the practice
population.
You
will be responsible for seeing and advising patients in respect of their
continuing medical and Pharmaceutical needs and developing your skills to see
and treat patients who wish to be seen on the day by a health care
professional; initially diagnosing and treating patients presenting with minor
illness. The Pharmacist practitioner will need to prioritise and triage the
needs of patients accordingly making any necessary referrals for investigations
in the appropriate manner. Patients with problems needing referral to secondary
care will be discussed with a GP before making a referral.
