Job summary
The Waterside Primary Care Network has an exciting
opportunity for a new Primary Care Network (PCN) Care Co-ordinator to join our PCN team!
You will join a great, diverse and multi-disciplinary team
engaged in cutting edge work to improve access to and experience of primary
care services, with the aim of improving our populations overall health.
This role will involve working at a PCN level
across our three local GP Practices (The Red & Green Practice, Waterfront
& Solent Surgery and Forestside Medical Practice) and therefore involves working with any patient within the Waterside area.
You will join a small but mighty, fantastic team of Care
Co-ordinators as well as being part of our Urgent Care Centre team based at Hythe Hospital.
Main duties of the job
- Checking patients in upon arrival at the UCC and enhanced access clinics
- Answering prescription queries from the patients or pharmacies
- Administrative tasks including scanning documents back to the patients registered practice and calling for ambulances to assist the Clinicians.

We are delighted to present an opportunity to join the Waterside PCN in an administrative role within our Urgent Care Centre (UCC).
Main duties of the role:
- Checking patients in upon arrival at the UCC and enhanced access clinics
