Job summary

We are looking for an experienced nurse who, within their professional boundaries, will provide autonomous care for patients within our PCN led Urgent Care Centre using history-taking and clinical assessment skills leading to diagnosis, treatment and evaluation of care. The Advanced Nurse Practitioner will demonstrate safe clinical decision-making and a high level of independent clinical care including assessment, diagnostic and prescribing skills for both urgent and long-term care.

Main duties of the job

  • Assess and treat patients, adult and children, presenting with undifferentiated and undiagnosed acute illness or long term conditions,
  • Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols and formularies;
  • Work with patients and their carers in order to support compliance with and adherence to prescribed treatments;
  • Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions;
  • Undertake telephone triage and consultations
  • Clinically assess, diagnose, plan, implement and evaluate interventions and treatments for patients with potentially complex needs;
  • To order diagnostic tests and initiate referrals as appropriate for patient needs;
  • To admit directly to hospital in acute medical need, undertaking a comprehensive handover;
  • To interpret findings or reports and take responsibility for following up your patients.
  • To contribute to the development of practice policies and procedures;
  • To assess the impact of policy implementation on care delivery;
  • Recognise and work within own competence and professional code of conduct as regulated by the NMC;

Red & Green Practice is an establish training practice on the edge of the New Forest and part of Waterside PCN. We have a large MDT of GP'S, NP, PN, Physiotherapist, Pharmacist, pharmacy technicians, Health & Wellbeing coaches & Care Navigators.

Role Overview

RGN1 with current NMC registration

MSc Advanced Nurse Practitioner

Independent Nurse Prescriber

A minimum of five years experience preferably in a Primary Care environment

Able to work as part of a team and autonomously

Excellent written and verbal communication skills

Good computer skills, specifically EMISWeb

Clean driving licence
