Job summary

Title: PCN Care Coordinator

Responsible to: Practice Manager

Location: Weavers Medical

Occasional working from home if required (COVID)

Another local practice as required from time to time

Hours per day: 4 x positions available

  • 32.5 hours per week M-F

  • 21.25 hours per week 4 days per week

  • 2 x 20 hours per week 4 days per week

    Salary: Starting salary £21,892 wte

    Main Purpose:

  • To provide coordinated care for all patients resident in care homes.

  • To maintain patient electronic records for clerical reasons, the Quality and Outcomes Framework and for medical care. The records must be accurate and contemporaneous.

Main duties of the job

Working as part of the Care Coordinator team to provide a single point of contact for all the Kettering Care homes for all non-urgent requests: dedicated phone line, nhs.net email address, Microsoft Teams.

Proactively contact all the care homes at least weekly to discuss patient related concerns.

Navigate the non-urgent requests and the patient related concerns to the most appropriate service or clinician

Contribute towards the creation and maintenance of the advanced care plans (including when appropriate frailty score, GSF level, date of last SMR, etc).

Ensure all information about the patient is updated using appropriate ARDENs templates and associated read codes.

Support alignment of care home to practices, including registration, updating care plans and liaising with clinical pharmacist to ensure medications are updated.

Weavers Medical is a teaching practice and hosts placements for GP Registrars and medical students for a number of universities.

Manage and prioritise workload on a daily basis and dealing with the competing demands of the care coordinator role.

Other Admin tasks

Attend regular team meetings.

Take part in any training as required from time to time.

Work to the schedule as set by the line manager and report any backlogs of work.

Any other duties considered relevant to the post.
