Job summary
We are seeking a dedicated and compassionate Chronic Disease Nurse to join our practice. The successful candidate will be responsible for providing high-quality patient care, supporting medical staff, and ensuring the well-being of patients across our 2 surgeries - Woolston Lodge and Chartwell Green. The role involves managing a caseload of patients, delivering clinics for chronic disease management, promoting selfcare, and supporting patients to achieve optimal health outcomes. The Nurse will work collaboratively with GPs, nurse practitioners, allied health professionals, and external agencies while maintaining autonomy within professional boundaries.

About us
Woolston and Chartwell Partnership is a large GP practice located on the east side of Southampton. The practice operates across two sites: Woolston Lodge Surgery and Chartwell Green Surgery, with the successful candidate expected to work across both locations. We currently serve a patient population of 22,000 registered individuals and are supported by a dedicated team of 85 staff members.

- Support doctors and other healthcare professionals during examinations and procedures.
- Maintain accurate and detailed patient records in accordance with legal and organisational standards.
- Educate patients and their families on health management, treatment plans, and post-discharge care.
- Assist in the organisation of patient flow to optimise efficiency within the healthcare facility.
- Ensure a clean, safe, and welcoming environment for patients and staff by adhering to health and safety protocols.
