Job summary

We are seeking a dedicated and compassionate Chronic Disease Nurse to join our practice. The successful candidate will be responsible for providing high-quality patient care, supporting medical staff, and ensuring the well-being of patients across our 2 surgeries - Woolston Lodge and Chartwell Green. This role offers an excellent opportunity for individuals committed to making a positive difference in patients' lives through professional and empathetic care.

The role involves managing a caseload of patients, delivering clinics for chronic disease management, promoting selfcare, and supporting patients to achieve optimal health outcomes. The Nurse will work collaboratively with GPs, nurse practitioners, allied health professionals, and external agencies while maintaining autonomy within professional boundaries.

About us

Woolston and Chartwell Partnership is a large GP practice located on the east side of Southampton. The practice operates across two sites: Woolston Lodge Surgery and Chartwell Green Surgery, with the successful candidate expected to work across both locations. We currently serve a patient population of 22,000 registered individuals and are supported by a dedicated team of 85 staff members.

Duties

  • Develop, implement, and evaluate personalised care plans in partnership with patients, families, and carers.

  • Ensure a clean, safe, and welcoming environment for patients and staff by adhering to health and safety protocols.

Qualifications

Essential

  • Full NMC Registration and ongoing revalidation
  • GCSE's grade A to C in English and Maths
  • Enhanced Disclosure and barring service clearance (DBS) carried out by the company

Experience

Essential

  • Proven experience in patient care within a clinical setting is desirable.
  • Strong knowledge of human anatomy and physiology.
  • Excellent communication skills with the ability to provide compassionate support to patients.
  • Ability to work effectively both independently and as part of a multidisciplinary team.
  • Organised, detail-oriented, with the capacity to handle multiple priorities efficiently.

Desirable

  • Experience working in Primary care
  • Mentorship, teaching experience
