Job summary
We are looking for an enthusiastic Advanced Nurse Practitioner /Paramedic Practitioner to join our friendly, forward thinking team to complement our existing clinical staff.
Main duties of the job
Beyond the expected assessment, treatment and collaboration as required, you will sometimes need to refer patients for investigations and/or the secondary care input, you will ensure the provision of continuity of care including follow-up if required. You will contribute to the writing of Care Plans for those patients at risk of hospital admission where necessary.
You will operate within your own sphere of competence and agreed acceptable limits of practice to provide expert professional clinical advice to patients, carers and colleagues, ensuring clinical safety and excellence.

We have an increasing patient population, and have demand for on the day consulting across all age groups. We also have a large house bound and care home population who require domiciliary consulting. We already have well established clinical pathways across our extensive MDT, so you would be supported by colleagues of different professions including MH Practitioners, social prescribers, pharmacists, as well as experienced GP's and existing ANP/Paramedics.
