Job summary

South Coast Medical Group is an award-winning NHS Primary Care Network, comprising five GP surgery locations in the Bournemouth general area and serving over 35,000 patients in total.

Crescent Providence Surgery, our founding Practice, has been rated Outstanding by CQC.

We are looking for a GP to join our team at Crescent Providence Surgery, Walpole Road. This is a 9 month fixed term contract to cover maternity leave. 7 sessions per week, Monday AM & OM, Tuesday AM, Wednesday AM & PM and Thursday Am & PM.

Main duties of the job

The post-holder will manage a caseload and deal with a wide range of health needs in a primary care setting. They must act within professional boundaries and provide the highest quality of holistic care for all patients. A successful candidate will be working alongside a friendly, hardworking team of general practitioners, nursing colleagues, admin and reception staff in the care of practice patients.

We have a designated Workflow team responsible for the receipt, circulation and management of all clinical correspondence, liaising between all disciplines of staff to provide support to our clinicians.

Job description

Job responsibilities

  • The post-holder will need to ensure they are readily available to work alongside the Practice timetable, undertaking a variety of duties including surgery consultations, telephone consultations and queries, visiting patients at home, checking and signing repeat prescriptions and dealing with queries.
  • Effectively manage paperwork and correspondence which need to be completed within a timely fashion.
  • Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within organisation.
  • Assessing the health care needs of patients with undifferentiated and undiagnosed problems.
  • Screening patients for disease risk factors and early signs of illness.
  • In consultation with patients and in line with current Practice disease management protocols, developing care plans for health.
  • Providing counselling and health education.
  • Admitting or discharging patients to and from the caseload and referring to other care providers as appropriate.
  • Recording clear and contemporaneous consultation notes to agreed standards.
  • Collecting data for audit purposes.
  • Compiling and issuing computer-generated acute and repeat prescriptions (avoiding hand-written prescriptions whenever possible).
  • Prescribing in accordance with the Practice prescribing formulary (or generically) whenever this is clinically appropriate.
  • In general the post-holder will be expected to undertake all the normal duties and responsibilities associated with a GP working within primary care.

File abnormal blood tests, as well as those for absent GPs include microbiology as these arent marked abnormal if positive.

File blood test for absent GPs in next 2 working days.

Deal with ALL DOCTORS tasks.

Complete scanned documents/workflow ALL DOCTORS group including necessary referrals/etc.

Attending training and events organised by the Practice or other agencies, where appropriate.

Filing pathology results and actioning related clinical work.

Filing of scanned documents/ workflow and the actioning of clinical work related to them.

Occasional home visits.

During busy periods including holidays & sickness you may be requested to see additional patients.

Duty Doctor Responsibilities:

Urgent telephone appointments.

Urgent Tasks.

Limited pre-booked telephone appointments to increase telephone capacity and for continuity (limited on busiest days, Monday and Friday).

Supervise/support urgent/same day queries pharmacy team, NPs, nursing team, frailty team

Supervise FY2, NPs.

Assist with triage/allocation of home visits Paramedics triage from 8.30-5.30.

See face to face appointments if urgent and all other appointments taken, often after paramedic triaged patient.

Patient services:

To provide medication review services to patients via clinics in the practice, domiciliary visits and in residential and nursing homes, and to deliver pharmaceutical care plans that maximise cost-effective prescribing and improve the quality of patient care.

To present at patient group meetings or other appropriate events to give advice on the appropriate use of medicines.

To produce patient information leaflets and posters and run medicine awareness projects throughout the year.

Assist Partners with the appropriate monitoring and management of their prescribing budgets.

To prepare evidence based resources and information to support the medicine management team and all other relevant health professionals in the implementation of rational cost-effective prescribing.

To help plan, develop and support the introduction of new working processes within the practice to optimise the quality of prescribing.
