Job summary

Do you want to help improve the health & wellbeing of Enfield residents?

Enfield Unity PCN are looking for an enthusiastic & dedicated health promotion care co-ordinator, to work with our GP practices to reduce health and social inequalities.

You will proactavly contact patients to advise on healthy weight, screen for hypertension and diabetes and promote vaccination.

Main duties of the job

  • Contact patients by text, phone, email and book review appointments, blood tests, check ups, immunisations and screening.
  • Obtain and record data such as smoking status, BP readings or BMI.
  • Work with clinical team to identify and contact patients in need of support.
  • Explain the principles of Stop Smoking Services so that patients can make an informed choice to stop smoking.
  • Motivate and empower clients to make behaviour changes.
  • In partnership with the service coordinator and manager develop new service initiatives.
  • Attend team meetings and training as required.
  • To actively participate in promotional events.

About us

Enfield Unity PCN is comprised of 21 sites.

Medicus Health Partners (MHP) is the lead practice for Unity PCN and will be the employing organisation. MHP is a super-partnership made up of 13 of the 21 PCN practices.

The successful applicants will have access to the training & support.

The successful applicants will be working closely with the PCN clinical directors and clinical leads.
