Job summary

We have an excellent opportunity available for an enthusiastic and motivated Clinical Prescribing Pharmacist to join our team. Working within this team, you would have the opportunity to work alongside GP Practices within the Grays Inn Medical Group.

Main duties of the job

Full time 40hrs per week

Medication review for repeat prescriptions including long term chronic conditions

Review and prescribe prescription medicines for registered patients

Liaise with doctors about pharmacy team regarding clinical cases and prescribing queries

Advise patients of any adverse side-effects of medicines or potential interactions with other medicines/treatments

* To ensure that prescriptions are up-to-date, revised and changed according need patient needs or changing condition acting promptly on requests from outside agencies

* To carry out medication reviews of patients on repeat medicines and amend prescriptions according to patient needs or changing conditions

* To add / amend medication in patient records from hospital letters

* To manage the repeat prescribing processes

* To carry out prescribing audits as required

* Attend and participate in any Practice meetings
