Job summary

The Paramedic Practitioner Triage Lead will be responsible for managing and delivering the daily triage function within the practice. This includes assessing, diagnosing, and treating patients with a range of acute and chronic conditions, ensuring patients are directed to the most appropriate care in a timely manner. The role will also involve service leadership, clinical supervision, and development of triage protocols in line with best practice and local pathways.

Main duties of the job

  • Lead and coordinate the daily clinical triage process, ensuring urgent and routine requests are prioritised appropriately.
  • Provide telephone and face-to-face consultations, home visits (where required), and urgent clinical assessments.
  • Support the GP team in managing on-the-day demand, streaming patients to the most appropriate clinician or service.
  • Act as a clinical lead for triage protocols and contribute to the development of practice systems to improve patient flow.
  • Mentor and support other clinicians in triage processes.

Job responsibilities

  • Clinical Duties

- Undertake clinical triage of all patient contacts (telephone, online, and walk-in) in accordance with practice protocols.

- Carry out telephone, face-to-face, and home visit consultations for acute and urgent presentations.

- Provide safe clinical decision-making, including independent prescribing (if qualified) or working to PGDs.

- Identify patients in need of urgent referral to secondary care or emergency services.

- Manage a caseload of patients with minor illness, minor injury, and acute presentations.

  • Leadership & Service Development

- Lead the daily triage rota and coordinate with GPs, nurses, and admin team to ensure smooth patient flow.

- Review and update triage protocols and ensure compliance with national and local guidance.

- Provide clinical supervision and training for colleagues involved in triage.

- Lead audits and quality improvement initiatives relating to urgent care and triage.

  • Teamwork & Collaboration

- Work closely with GPs, nurses, pharmacists, and social prescribers to deliver holistic patient care.

- Liaise with local urgent care providers, district nursing, and community teams.

- Participate in MDT meetings and case reviews.

  • Professional Development

- Maintain HCPC registration and keep up-to-date with evidence-based practice.

- Participate in annual appraisal and CPD activities.

- Engage in reflective practice and learning from significant events.
