Main duties of the job

The Clinical Performance Coordinator plays a pivotal role in enhancing patient care by developing, implementing, and monitoring clinical services in compliance with national and local directives. The role focuses on improving service efficiency, ensuring high-quality, evidence-based primary care, and optimizing digital tools for the best patient experience.

We are a well-established NHS GP surgery located in Clerkenwell, London EC1, around the corner from London City University and the renowned Exmouth Market, described as "one of central Londons most charming spots" by London TimeOut.

Our friendly and dedicated team includes doctors, nurses, pharmacists, administrators, and other healthcare professionals, all working together to care for approximately 17,000 patients. Job description

Job responsibilities

Clinical Performance & Staff Relationship Management

Oversight of all planned care processes, including long-term conditions management, public health programs, and immunisation schedules.

Ensure all clinical staff, including doctors, nurses, ARRS roles, and healthcare assistants, effectively use EMIS clinical templates to record patient outcomes.

Performance management of healthcare assistants to ensure the efficient delivery of planned care, both through appointment scheduling and patient recalls.

Lead on the most cost-effective recall mechanisms, prioritizing digital methods but ensuring telephone and letter options when needed.

Develop and implement strategies to support non-digitally proficient patients, ensuring equitable access to chronic disease management services.

Lead on clinical risk stratification, adopting new tools to guide patients to the best care pathways while prioritizing continuity of care for those who need it most.

Work with the team from South Islington Primary Care Network to ensure delivery of PCN targets.

Digital Tools & IT Responsibilities

Ardens Manager: Ensure effective use of Ardens templates and reporting tools to optimize clinical performance.

Digital Tools for Patient Care: Maximize the use of iPlato, Accurx, Spryt, and eConsult for digital invitations and data collection.

Online Consultation Management: Oversee the use of eConsult to streamline planned care and enhance patient engagement.

EMIS Web: Maintain and develop templates, protocols, and referral forms to improve clinical efficiency.

Access Control & IT Support: Ensure all staff have appropriate access to clinical and operational systems, maintaining an up-to-date access register.

Data Management & Reporting: Regularly perform clinical searches for CQRS, invoicing, and evidencing service performance.

Website & Digital Communications: Ensure practice website, NHS Choices, and other media channels are accurate and up-to-date.

Quality Monitoring & Compliance

Conduct regular audits to ensure accurate clinical coding and high-quality performance monitoring.

Lead on the effective recall process, optimizing scheduling and outreach strategies.

Work with external agencies and networks to maximize practice income and service quality.

Review and update policies, protocols, and procedures in line with national guidelines.

Support the pharmacist team to ensure all safety alerts, including MHRA notifications, are actioned and implemented.

Communication and Team Collaboration

Act as a key liaison between clinical and administrative teams to ensure alignment on clinical performance goals.

Attend and contribute to weekly operational meetings, providing data-driven insights and reports.

Lead or coordinate Friends and Family feedback collection, ensuring patient insights inform service improvements.

Support significant event and clinical complaint investigations, identifying areas for improvement.

Contribute to the induction program for new staff to ensure consistency in performance expectations.
