Main duties of the job
As
part of our Long Terms Conditions team, including 3 LTC specialist nurses, you will be responsible for areas such as health promotion,
chronic disease management and health prevention.
You
will ideally be a nurse prescriber with experience within a primary care setting, have
excellent clinical skills and be competent in pro-active management of patients
with chronic diseases including Diabetes, Asthma, COPD, Hypertension and
Cardiovascular disease.

We are 2 site, dispensing practice in Kendal with around
17,500 patients keen to attract caring, motivated, and passionate colleagues.
We offer a wide range of enhanced services, plus Dermatology and ENT clinics.
We are a GP registrar and medical student training practice
with an active frailty and care home support team. We are an accredited Veteran
friendly practice, a Park Run practice and are proactive in women's and men's health.
