Main duties of the job

As part of our Long Terms Conditions team, you will be responsible for areas such as health promotion, chronic disease management and health prevention.

You will ideally be a nurse prescriber with experience within a primary care setting, have excellent clinical skills and be competent in pro-active management of patients with chronic diseases including Diabetes, Asthma, COPD, Hypertension and Cardiovascular disease.

About us

We are 2 site, dispensing practice in Kendal with around 17,500 patients keen to attract caring, motivated, and passionate colleagues. We offer a wide range of enhanced services, plus Dermatology and ENT clinics.

We are a GP registrar and medical student training practice with an active frailty and care home support team. We are an accredited Veteran friendly practice, a Park Run practice and are proactive in women's and men's health.

We are part of an excellent Primary Care Network and supported with clinical services from a wide range of disciplines. We offer
