To work as part of a multi-disciplinary team in a patient facing role to clinically assess and treat patients using your expert knowledge of medicines for specific disease areas.
To be a prescriber, or completing training to become a prescriber, and work with and alongside the general practice teams.
To be responsible for the care management of patients with chronic diseases and undertake clinical medication reviews to proactively manage people with complex polypharmacy, especially the elderly, people in care homes, those with multiple co-morbidities (in particular frailty, COPD and asthma) and people with learning disabilities or autism (through STOMP Stop Over Medication Programme).
To provide specialist expertise in the use of medicines whilst helping to address both the public health and social care needs of patients at the PCNs practice(s) and to help in tackling inequalities.

Mandatory registration with General Pharmaceutical Council

About us
Liverpool First are an established Primary Care Network who are now looking to grow their team of Clinical Pharmacists. The Network have benefited from a working with a Network Clinical Pharmacist since August 2019 and are now looking to directly employ a further two to strengthen the team.
Liverpool First Primary Care Network is made up of 8 practices in Anfield, Everton and Old Swan, Walton and Kensington and covers a population of 38,000. The Clinical Pharmacists will be employed by one of the practices in the network and will be required to work across all 8 practice sites within the Network.

KEY RESPONSIBILITIES

To be responsible for the care management of patients with chronic diseases and undertake clinical medication reviews to proactively manage people with complex polypharmacy, especially the elderly, people in care homes, those with multiple co-morbidities (in particular frailty, COPD and asthma) and people with learning disabilities or autism (through STOMP Stop Over Medication Programme);
d. To provide specialist expertise in the use of medicines whilst helping to address both the public health and social care needs of patients at the PCNs practice(s) and to help in tackling inequalities;
e. To provide leadership on person-centred medicines optimisation (including ensuring prescribers in the practice conserve antibiotics in line with local antimicrobial stewardship guidance) and quality improvement, whilst contributing to the quality and outcomes framework and enhanced services;
f. Through structured medication reviews, support patients to take their medications to get the best from them, reduce waste and promote selfcare;
g. To have a leadership role in supporting further integration of general practice with the wider healthcare teams (including community and hospital pharmacy) to help improve patient outcomes, ensure better access to healthcare and help manage general practice workload;
h. To develop relationships and work closely with other pharmacy professionals across PCNs and the wider health and social care system;
i. To take a central role in the clinical aspects of shared care protocols, clinical research with medicines, liaison with specialist pharmacists (including mental health and reduction of inappropriate antipsychotic use in people with learning difficulties), liaison with community pharmacists and anticoagulation
COLLABORATIVE WORKING
- To work as part of the multidisciplinary team, to provide advice and support on all prescribing issues. When clinically appropriate, to implement and action changes to the patients medical prescribing record as directed by primary, secondary or community care; working at all times within professional limits.
- To influence the prescribing behaviour of all members of the Practice MDT including GPs and practice nurses
- Liaise with health professionals in secondary care on interface medicines management issues.
- Ensure prescribing and administration of medicines and disposal of medicinal waste follows appropriate legislation and health and safety policies.
- To promote high quality, safe and cost effective prescribing as informed by evidence based practice and national and local guidance etc. To ensure prescribing conforms to relevant legislation.
- Working with the Liverpool First Primary Care Network GP Pharmacy Lead & Clinical Director in agreeing and delivering the Work Plan for your allocated practices in addition to directing and advising on medicines management issues in general.
- To work collaboratively with the wider network pharmacist team
- To become an integral part of your network team and to undertake those clinical and administrative duties as determined by shared agreement with the Lead GP
- To initiate & complete medication and prescribing related audits. To develop a systematic timetable for regular audit as and when required. To present audit for discussion at Practice MDT meetings and when agreed, implement the relevant actions
