Job summary
Cricklewood Health Centre is located in a newly
refitted premises close to Cricklewood Station. We have a growing list, and
currently serve a patient population of 5000.
We are looking for an outstanding Practice Nurse to
join our team for 16 to 25 hours per week and we can be flexible with the
working pattern.
Applicants should be NMC registered and either
currently working as practice nurse, or working in a hospital setting and looking
to transfer to primary care.
Our ideal candidate will be looking to develop a

Main duties of the job
The purpose of this role is to deliver efficient
and effective nursing care to patients attending the surgery, providing all the
normal duties of a practice nurse including:
Immunisations and Vaccinations
Chronic Disease Management
Cervical Cytology
Wound Management
Diabetes and NHS Health Checks
Responsibility for Infection Control and Cold Chain
Management

POST TITLE: Practice Nurse
RESPONSIBLE TO: Lead GP
HOURS: Flexible, between 16 and 25 hours
SALARY: competitive and negotiable depending on experience
HOLIDAY: 5 weeks plus bank holidays + 1 week study leave (pro rata)
The purpose of this role is to deliver efficient and effective nursing care to patients attending our practice.
Duties
- Maintain accurate records of care, consultation and medication administration, accurately monitoring and record observations as required.
- Identify patient care planning needs and provide support and advice for long term conditions
- Implement Health promotion and well-being programmes
- Manage patients presenting with a range of acute and chronic medical conditions, providing subject matter expert advice
- Develop and implement individual care plans for chronic disease patients, including providing LTC clinics and refer patients to secondary care as required
- Monitor patients' diet and exercise, including providing NHS Health Checks and reviews as required
- Working with the practice team to support the development, delivery and review of the surgery action plan including list growth, patient satisfaction levels, QOF, enhanced services, KPI targets, resource profile and cost management.
- Provide wound care
- Provide specialist clinics such as ear syringing, contraception etc.
- Collect specimens for pathology
- Provide travel advice and vaccinations
- Provide cervical cytology for the practice
- Process and interpret test results as required (e.g. cervical cytology results)
- Ensure data is coded appropriately
- Implement an effective immunisation programme in line with the national schedule
- Implement vaccination programmes for adults and children
- Support patients in the use of their prescribed medicines or over the counter medicines (within own scope of practice)
- Liaise with external services and agencies to ensure vulnerable patients are supported (safeguarding)
- Refer patients according to practice/local policies for substance abuse
- Support the clinical team with safeguarding matters in accordance with local and national policies
- Delegate clinical responsibilities as necessary and where appropriate (ensuring safe practice and the task is within the scope of practice of the individual)
- Provide nursing expertise and guidance within the practice as necessary
- Act as the infection prevention and control lead for the practice, ensuring compliance with all relevant legislation and regulations, undertaking periodic audits as necessary
- Monitor and ensure the safe storage, stock rotation and disposal of emergency drugs and vaccines.
- Manage stocks of supplies and provide cold chain management
- Ensure storage and disposal of medication is carried out according to policy
