Main duties of the job

We are looking for a candidate who has a passion for nursing and the enthusiasm to undertake the training and development required for this role.

You will need to be a team player and be able to demonstrate excellent communication skills, both with individual patients and the wider multi-disciplinary practice team.

You will have a minimum of two years nursing experience post registration. Broader experience of primary care would be really helpful.

About us

Our Practice is based over two sites close to the Yorkshire / Lancashire border in Colne and Earby. We have a patient list of over 15,000 patients. We are a training practice and have a strong emphasis on working as a team to provide high quality patient-focussed care.

We are keen to develop new ways of working, and see our Nursing team as having a major role in the future of our wider practice team.

We take an active role in the Pendle East Primary Care Network and work closely with our wider Pendle East Neighbourhood Team.

As a result of the strengthening relationships with other Practices locally, we will be working with another Practice within our group to ensure we can provide successful candidates with the support, development and experience they need for a successful career in practice nursing.

Hours 24 hours per week (including some Saturday morning and possible evening working)

Location The post will be based at both our sites in Earby and Colne with opportunities to link with a neighbouring GP Practices in our PCN.

Job Summary

To provide high quality, effective, evidence-based nursing care to meet the needs of our registered practice population.

The post is recognised as a development post for registered Nurses moving into Practice Nursing. Successful candidates will not be expected to undertake the full range of duties without support and training.

The post will focus on the pro-active management of long-term conditions as well as assessment, screening, preventative care, QOF and audit. The postholder will be a key member of our Nursing team and multi-disciplinary Practice team.

Responsibilities

1. To assess, plan, provide and evaluate nursing care to meet the care needs of individuals and groups in our practice population.

2. To pro-actively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition.

3. To manage and treat long-term conditions, working with other healthcare professionals where appropriate, and in accordance with evidence-based practice, as well as national and practice protocols.

4. To provide general nursing care on the assessment and management of minor ailments/illness and general health problems.

5. To carry out cervical cytology tests in line with national procedures.

6. To be involved in the delivery of all vaccination and immunisation services adult and childhood service, including flu and travel.

7. To provide support for our Health Care Assistants with blood tests, ECGs, blood pressure checks, new patient checks, 24 hour blood pressure monitoring, and stock control

8. To work to ensure all necessary QOF targets and other local quality targets are met as far as possible.

9. To be responsible for the review and implementation of infection control measures.

10. To contribute to the development and review of practice protocols.

11. To implement, support and manage specific public health/health care projects, e.g. Flu campaign, Hib vaccination, Smoking Cessation.

12. To support practice development plans and other local and national initiatives

13. To contribute to research and audit as required

14. To take part in our Practice annual appraisal process, and undertake revalidation when required.

15. To take responsibility for your own learning, development and performance and ensure that training needs are identified and discussed as appropriate.

16. To ensure mandatory training requirements are met on a regular basis.

17. To undertake any other duties within the professional and general competence of the post holder, as required by the Practice.

General duties

18. To communicate effectively with patients, carers and all members of the Practice Multi-disciplinary team and to maintain a caring environment through the support of colleagues.

19. To ensure accurate medical records are maintained at all times, which are consistent with legislation, policies and procedures.

20. To be responsible for the management of equipment and resources as part of the Nursing Team. This may involve for example, the ordering and maintenance of equipment / instruments / disposables, storage of vaccinations, drugs and other clinical stock, and ensuring that medical devices are calibrated, PAT tested and serviced where appropriate.

21. To support education and training for other staff members, medical students and others as required.

22. To provide holiday cover for other practice nurses as required

23. To attend staff meetings as requested.

24. To take care of your own safety and that of other people who may be affected by your own act or omission.

25. To ensure current training requirements in relation to Safeguarding are up-to-date, and to consider aspects of Safeguarding throughout all elements of working practices.

26. To ensure that any safeguarding concerns are reported to the GP Safeguarding Lead, or their Deputy, within the practice.

- Identifying, reporting and correcting health and safety hazards and infection hazards when recognised

- Awareness of national standards of infection control and cleanliness

- Keeping own work areas and general / patient areas clean and safe
