Job summary

The Pendle Medical Partnership has an exciting opportunity for an enthusiastic and committed Practice Nurse to join our friendly team. Prior knowledge of primary care would be helpful but not essential as we are a great team of clinicians who work in a fully supportive way. The successful candidate will work part time typically 25 -28 hrs over 3 to 4 days as a member of our practice team across two sites, Earby and Colne.

Primary care roles are not typically full AfC Contracts, however Pay is negotiable and closely aligned in the range of NHS PB 5 -7 depending on experience and clinical skill sets.

Main duties of the job

You will be a dedicated, skilled practice nurse, with a passion for providing the highest quality of care for our patient population of around 15,250 patients. The successful applicant will have a wide range of experience and knowledge with different long term health conditions. It is ideal if applicants have a background in primary care and a good understanding of QOF outcomes, but not essential as we are a great team of clinicians who work in a fully supportive way.

What is more important to us is flexible, hard working and a patient focussed, caring nurse with a great sense of humour and outgoing personality.

About us

The Pendle Medical Partnership cares for approximately 15,000 patients in the Pendle area, this is the result of a formal merger between two successful GP practices located on the Lancashire / Yorkshire border. Both Edward Street Surgery and Colne Corner Surgery are long-established practices with a strong emphasis on providing high quality, patient-centred care. Working across two sites and with a staff team of 50+ , we ensure that a local, family feel to our Partnership is achieved whilst maintaining a professional service at all times.

We currently have 5 partners, 5 salaried GPs, 2 PA's, 10 nurses & 6 HCA's working a range of flexible full and part time roles. All our team play a vital and integral part of our patient care. The role will work across BOTH sites on agreed shift patterns.

Job Summary

To deliver high quality primary health care services for patients within the practice. You will be an experienced practice nurse that works to undertake all nursing care in accordance with NMC. The successful candidate will work collaboratively with all members of the multi-disciplinary team in order to promote a high standard of health and well-being amongst the patient population.

Responsibilities

1. Assess, plan, provide and evaluate nursing care to meet the care needs of individuals and groups in the practice population, managing own caseload.

2. Meeting all necessary QOF targets and maintaining accurate records.

3. Manage care programme for patients with acute and chronic diseases including groups such as; Diabetes, Womens Health, CHD, Asthma, COPD and Hypertension.

4. To provide general nursing care on the assessment and management of minor ailments/illness and general health problems. This includes support of HCA/phlebotomist role with blood tests, ECGs, blood pressure checks, new patient checks, 24 hour blood pressure monitoring, stock control.

5. To be responsible for the review and implementation of infection control measures.

6. To be responsible for the planning, revision and implementation of all vaccination and immunisation services adult and childhood service, including flu and travel.

7. To contribute to the development and review of practice protocols.

8. To implement, support and manage specific public health/health care projects, e.g. Flu campaign, Hib vaccination, Smoking Cessation.

9. Develop, implement and manage health promotion clinics.

10. To work as part of the practice nursing team to ensure that the whole population within our area is given the highest level of care.

11. Support practice development plans and other local and national initiatives

13. To provide holiday cover for other practice nurses as required

14. To attend staff meetings as requested by the practice manager.
