Job summary

West Enfield Collaborative Primary Care Network has an exciting opportunity for a Practice Pharmacist to join its PCN based in North London providing high-quality care to around 40,000 patients. The PCN covers three GP sites two of which are approved training practices. We are interested in candidates who are already fully functioning Pharmacist who are looking for an opportunity to broaden their horizons, get more autonomy, and take on more clinical responsibility.

Your role will require you to participate in all aspects of patient care through knowledge gained from your training.

Main duties of the job

Responsible for supporting the partners in implementing effective medicine management within the practice, identifying areas for improvement, and initiating and managing change.You will be working as part of an allied healthcare professional team including Pharmacists, Paramedics, Physicians Associates, Nurses and Healthcare Assistants, and will be supported and supervised by a GP.

We are a PCN in North London (Enfield) providing high quality care to around 40,000 patients. The PCN covers three well established GP sites two of which are approved training practices. We believe in working collaboratively and have regular team meetings. Our workforce include GPs, GP and FY2 trainees, Nurses, Pharmacists, Social Prescribers, Care Coordinators, and full complement of administrative staff. You will be employed by the PCN and your time will be divided between one or more site depending on hours worked and PCN needs.

Job description

Job responsibilities

  • Develop and manage a medicines management plan and deliver patient services as determined by practice policy and local and national guidance.
  • Provide expertise in clinical medicines review and address public health and social needs of patients in GP practices.
  • Reduce inappropriate poly-pharmacy and wasteful prescribing through clinical medication review.
  • Reconcile medicines following hospital discharge and work with patients and community pharmacists to ensure patients receive the medicines they need post-discharge.
  • Contribute to reductions in medicine-related hospital admissions and readmissions by supporting patients to get the best outcomes from their medicines and identifying and addressing medicines-related issues.
  • Run clinics where medicines are the main interventions e.g. anticoagulant monitoring clinics.
  • Contribute to multi-morbidity reviews and multidisciplinary reviews.

  • Undertake minor ailments clinics and triage.
