We are looking for
an enthusiastic, flexible, dedicated, and self-motivated, forward thinking ANP
to join our team. Ideally, we are looking for 3 days per week. The job is an
ideal opportunity for someone seeking to deliver truly patient-centred care whilst
developing the ANP role to its full potential in a supportive environment. You would become a valued member of our team

rural environment.
We recognise the
importance of a balanced work-home life and our clinicians
have 15-minute
appointments and dedicated administration time with opportunities to pursue
special interests.
Main duties of the job
You will be an
autonomous practitioner working independently and in conjunction with other
health care professionals within their professional scope of practice providing
care for patients face to face and telephone consultations. You will assess,
diagnose, and treat patients with a range of minor illness, acute and chronic
medical conditions and initiate direct referrals as appropriate. You
will also be comfortable analysing and interpreting patient test results

7 appointments (AM)
of 15 minutes each (telephone call and face to face) or covering the open
surgery (AM) 9 face to face appointments of 10 minutes each followed by a
telephone triage list

About us
A unique
opportunity to work in a small friendly rural GP practice with traditional
values delivered in a modern healthcare setting. We are a dispensing practice
and have a list size of approximately 4500 patients. Sedbergh is a small town within the Yorkshire
Dales National Park, in an area of outstanding natural beauty and set within a
stunning back drop of the Howgill Fells. Sedbergh is home to the prestigious
Sedbergh School, whilst being only 10 minutes drive from the M6 J37.
We are a close-knit
team dedicated to improving health and well-being in our community. In addition to the 2 partners, our clinical
team includes 3 part time salaried GPs, 3 Practice Nurses, an HCA and
Phlebotomist. Our dedicated non-clinical
team includes a Practice Manager along with a brilliant team of receptionists
and a team of dispensers.
The Practice is
part of the Western Dales Primary Care Network and has strong links with
neighbouring practices and we anticipate these developing further over the
coming years. We already benefit from the ARRS roles with a clinical
pharmacist, Social Prescriber, First Contact Physiotherapist, Mental Health
Practitioner, HAWC, and a Nurse Associate.
We always try to
work smartly and currently use various systems including EMIS, AccuRx and
Clarity TeamNet to ensure we are paper light and efficient.

Clinical Nursing Practice
Work as part of a multi-disciplinary
team carrying out clinical nursing practice at an advanced level using expert
clinical skills, critical judgements & knowledge to deliver appropriate high
quality and safe care.
Delivery of care in line with NSF, NICE
guidelines, and evidence-based care, including Practice Standard Operating
Procedures.
Triage and treat patients, performing
specialist assessment of their needs and make referrals as necessary to other
members of the primary and secondary health care teams as appropriate.
Co-ordinate the planning and delivery
of care ensuring patients and carers/relatives are involved.
Prescribing and reviewing medication
that is appropriate to patient needs and in accordance with latest
evidence-based practice, national and practice protocols and within the scope
of qualified practice.
Request diagnostic investigations and
interpret within agreed protocols e.g., x-ray, blood tests, scans, etc.
Refers patients to other members of the
multi-disciplinary practice teams, secondary care organisations and other
organisations as necessary.
Administration and professional
responsibilities
Produce accurate, contemporaneous, and
complete records of patient consultation, consistent with legislation, policies,
and procedures
Participate in the administrative and
professional responsibilities of the practice team including attending practice
meetings as required
Organise and plan the provision of
nursing services for the practice to include the delegation of duties to other
members of the practice nurse team as appropriate
Contribute to local and national
practice targets clinical remit e.g., QOF, prescribing incentive scheme,
National benchmarking.
Training and personal/professional
development
Maintain up to date skills and
knowledge, maintaining awareness of professional issues at an advanced level.
Training needs will be monitored by
yearly appraisal and will be in accordance with practice requirements. Personal
development will be encouraged and supported by the practice.
Fully engage in practice team education
events and practice development.
Contribute to the identification and
assessment of learning needs of staff and other professionals and assist in
planning effective programmes of education.
Act as a mentor for junior staff,
assessing competence against set standards.
Develop and maintain a Personal
Learning Plan.
Regularly participate in clinical
supervision.
