Job summary
An exciting opportunity has arisen to join our
multi-disciplinary PCN team which consists of 8 GP Practices, Clinical
Pharmacists, Paramedics, Social Prescribers, Physiotherapists, Care
Coordinators, Health Coach and Pharmacist Technicians.
The successful candidate will work 4 sessions (2 days) per week (ideally working on two of the following days: Monday, Tuesday, Friday)
Due to the changes in the ARRS Scheme to introduce General
Practitioners we are looking to expand our team by seeking to employ a highly
motivated, newly qualified General Practitioner to work alongside our practices
and PCN staff.
PLEASE NOTE:
This role is for Stockport East and South PCN and you will be employed by a practice within the PCN.
The post holder will need to have full access to a car to enable full geographical working across Stockport East and South PCN (Hazel Grove to High Lane, Marple, Romiley and Woodley)and work flexibly to meet the needs of the PCN.
This is a permanent contract subject to the funding related to the ARRS scheme.

Main duties of the job
To perform as an autonomous general practitioner working
independently and with other health care professionals to assess, diagnose and
treat the conditions of patients attending within a primary care setting, both
in practice and on home visits.
Provide expert professional advice to patients, carers and
colleagues and ensure the maintenance of clinical excellence. To be
professionally accountable and responsible for all elements of clinical
practice in accordance with the GMC code of conduct ensuring the safety of
patients and quality of their care is primary.

needs of our patients. We are a high achieving PCN performing well across the
board with consistently positive friends and family feedback, leading in covid
and flu vaccinations and managing prescribing targets well when compared with
Stockport as a locality and Greater Manchester as an ICB.
We are 8 GP Practices in Stockport East and South PCN. We
have a weighted list size of approximately 60,000 patients between the 8 GP
practices. Our practices look after patients from Hazel Grove to High Lane,
Marple, Romiley and Woodley.

Job responsibilities
Clinical responsibilities
- In accordance with the practice rota, as agreed,
the post-holder will make him/her-self available to undertake a variety of
duties including surgery consultations, telephone consultations and queries,
visiting patients at home, checking and signing/authorising repeat
prescriptions and dealing with queries, paperwork and correspondence in a
timely fashion.
- Making professional, autonomous decisions in
relation to presenting problems, whether self-referred or referred from other
health care workers within the organisation
- Assessing the health care needs of patients with
undifferentiated and undiagnosed problems
- Screening patients for disease risk factors and
early signs of illness
- In consultation with patients and in line with
current practice disease management protocols, developing care plans for health
- Providing counselling and health education
- Referring to other care providers as appropriate
- Recording clear and contemporaneous consultation
notes to agreed standards
- Compiling and issuing computer-generated acute
and repeat prescriptions
- Prescribing in accordance with the practice
prescribing formulary (or generically) whenever this is clinically appropriate
- The post-holder will be required to visit
patients during surgery time in an emergency if clinically indicated.
- The post-holder will participate in the extended
hours enhanced service.
- The post-holder will undertake fair share of
home visiting sessions per week as allocated.

agencies) in order to assure appropriate care is provided for the Practice
population

patients and carers, recognising their needs for alternative methods of

communication styles to ensure patients are fully informed and consent to
