Role Summary:
To support our practices and ensure patients receive the best quality care West Leeds PCN is seeking an experienced Diabetic Nurse Specialist to work alongside our Senior Clinical Pharmacist and HF Nurse, the Frailty Team, and clinicians within practices to develop and deliver a comprehensive Diabetic service for the patients within the PCN.
To be responsible for the management of diabetes including insulin initiation, health education, reviewing bloods, medication monitoring and dose adjustment. Working as part of the PCN multidisciplinary team, delivering care within their scope of practice to the entitled patient population. The role will include supporting the management team in the reviewing of clinical policy and procedure.
The post holder will deliver clinic and home-based treatment plans including education and lifestyle advice, monitoring and management of symptoms and medications.
The service seeks to provide access to diagnostics and early treatment intervention. The post is integral to the multidisciplinary team, working closely with our partners in secondary care to provide seamless transition for patients to primary care underpinned by a solid clinical governance structure. The post holder will build relationships with wider external organisations to promote joined up patient care and services.

Key Responsibilities
Responsible for the assessment, planning and implementation of clinicalmanagement plans.
Deliver direct care to patients including providing personalised planned interventions to support people to remain safe, well and as independent as possible in their own homes.
Initiate insulin and proactively monitor the care of all diabetic patients in line with local and national policy.
Provide education and training to health care professionals across the pathway and developing the focal point for responsive, high quality, evidence based clinical advice in the out of hospital system, closely integrated with secondary care specialities.
Works in partnership with other groups in the development of innovative clinical practice.
Assists in managing a caseload, in line with PCN policies, guidelines and national standards.
Promotes and enables patient choice and independent living.

Primary Care Networks (PCNs) form a key building block of the NHS long-term plan. Bringing general practices together to work at scale has been a policy priority for some years for a range of reasons, including improving the ability of practices to recruit and retain staff; to manage financial and estates pressures; to provide a wider range of services to patients and to more easily integrate with the wider health and care system.
West Leeds Primary Care Network (WLPCN Ltd) is a large PCN with a patient list of circa 70,000 patients around the Pudsey / Bramley area. We have a significant workforce in place which brings wonderful opportunities for in-house multi-disciplinary team working. We have a well developed multidisciplinary team including a Pharmacy Team, Occupational Therapists, Heart failure nurse specialist, Social Prescribers, Paramedics, Dietitians, a range of nursing / HCA staff and more which means that the majority of any patient's needs can now be provided on their own doorstep.
We have a strong ethos of training and development and host 3rd & 4th year medical students and GP registrars. As a well established and mature PCN we have a strong culture of governance and clinical supervision. We have a GP educational lead, A CVD lead, a Lead Practice Nurse and a Lead Practice manager, who, along with the Clinical Director form the PCN executive.
