The Groves Hinchley Wood is seeking a motivated Practice Nurse to join our team and provide highquality care to patients at our Hinchley Wood Surgery.

We are looking for a qualified General Practice Nurse, and nurses with relevant experience who are keen to develop their skills in primary care are encouraged to apply.

This is a part time role, 3 days per week -we are open to discussing flexible working arrangements for the right candidate.

Main duties of the job

As a practice nurse you will

  • conduct comprehensive assessments of patients health needs

  • develop implement and review individualised care plans

  • provide appropriate treatments and interventions as required

  • undertake treatment room responsibilities and deliver chronic disease management clinics

  • carry out specific clinical tasks including but not limited to

    • travel health consultations and vaccinations

    • cervical screening

    • childhood immunisations

    • wound management

    • blood pressure monitoring and ecgs

    • venepuncture and health checks

  • work collaboratively with gps other nurses and the wider healthcare team to ensure high quality patient care

  • maintain accurate timely and confidential patient records in line with nhs standards

  • deliver health education advice and counselling to patients and their families

  • keep up to date with current clinical guidelines medical advancements and best practice in primary care

This is a parttime position worked over three days per week, Monday to Friday, with flexibility available for the right candidate. The role includes a broad range of clinical responsibilities, such as patient assessments, care planning, treatment room duties, and the delivery of Chronic Disease Management clinics. You will also undertake specific tasks including travel health consultations, cervical screening, childhood immunisations, wound care, venepuncture, and routine health checks.

Working closely with GPs, fellow nurses, and the wider healthcare team, you will play a key role in ensuring patients receive safe, effective, and compassionate care. You will maintain accurate patient records, provide health education and counselling, and stay up to date with current clinical guidelines and best practice.

  • Evidence of uptodate NMC revalidation -
  • Experience in primary care GP setting -
  • Competence in adult and childhood immunisations and vaccinations -
  • Cervical cytology training -

  • Experience in wound care and dressings -
  • Ability to manage Chronic Disease Management clinics such as diabetes, asthma and COPD -
  • Good IT skills and confidence using electronic patient record systems

Desirable

  • Travel health training and vaccination experience -
  • Additional qualifications in chronic disease management such as diabetes or respiratory training -
  • Evidence of ongoing professional development including CPD courses or certificates -
