Job summary
Whitstable Medical Practice is looking to recruit a Care
Coordinator to support our Frailty Advanced Clinical Practitioners (ACPs).
The post will be for 37 hours per week - work pattern to be confirmed. We offer a competitive salary as well the NHS pension scheme.
Main duties of the job
The successful candidate
will play a key role in proactively identifying and working with people,
including the frail/elderly and those with long-term conditions, to provide
coordination and navigation of care and support across health and care
services.
They will work closely with the Frailty ACPs and GPs making
sure that appropriate support is made available to people; supporting them to
understand and manage their condition and ensuring their changing needs are
addressed. They will enable people to access the services and support they
require to meet their health and wellbeing needs, helping to improve peoples
quality of life.
The successful candidate will have excellent administrative, interpersonal and communication skills, be organised, patient and empathetic.
They will have experience of working in health, social care or other support
roles including direct contact with people, families or carers.
About us
Whitstable Medical Practice is comprised of three sites
and makes up is its own Single Practice Primary Care Network. The practice
provides healthcare services to over 40,000 patients in the area. As is typical
across the country, we are experiencing an increasing elderly population which
is placing additional pressures on the local health economy. There are 10 care
and nursing homes in the area all of whom are registered with the practice

The care coordinator plays an important role to
support and work with older people living with frailty to provide coordination
and navigation of care and support across health and care services.
Their work supports the Frailty ACPs, GPs and
practice teams to manage a caseload of patients, acting as a central point of
contact to ensure appropriate support is made available to them and their
carers.
The care coordinator will review patients’ needs
using the personalised care plans written by the Frailty ACP’s to help them
access the services and support. This
may include referring to social prescribers, health and wellbeing coaches and
other professionals or services where appropriate. Their aim is to help people improve their
quality of life.
The care coordinator will provided administrative
support for the frailty team including making appointments, carrying out follow
up phone calls, triaging new referrals and incoming calls and correspondence.
This role will become an integral part of Whitstable
Medical Practice’s Frailty team as well as working alongside our social
prescribers to provide an all-encompassing approach to personalised care.

- English and Maths to GCSE level C or above or equivalent
- Demonstrate commitment to professional and personal development

- Experience of working with health sector and multiple stakeholders
- NVQ Level 3 or above

- Experience of working in health, social care and other support roles in direct contact with people, families or carers (in a paid or voluntary capacity)
- Experience of working within multi- professional team environments
- Experience of supporting people, their families and carers in a related role
- Experience of data collection and using tools to measure the impact of services
- Experience providing administrative support in a health or social care setting
Desirable
- Experience of working directly as a care coordinator or equivalent role
- Experience or training in personalised care and support planning
- Experience of working with older people and people with a dementia diagnosis

- Understanding of the wider determinants of health, including social, economic and environmental factors and their impact on communities, individuals, their families and carers

- Knowledge of how the NHS works, including primary care
- Ability to recognise and work within limits of competence and seek advice when needed
- Understanding of the needs of older people / adults with disabilities / long term conditions particularly in relation to promoting their independence
- Basic knowledge of long term conditions and the complexities involved: medical, physical, emotional and social
- Understanding of the needs of older people / adults with disabilities / long term conditions particularly in relation to promoting their independence
- Established IT skills and experience in using Microsoft Excel, Word, Outlook
