Job summary

The post holder will provide general medical services managing a caseload and deal with a wide range of health needs in a primary care and UTC setting, ensuring the highest standards of care for registered and temporary patients.

Main duties of the job

This is a blended role working as part of a team combining primary care (GMS) and UTC sessions that support the partnership manage a busy clinical caseload. The appointee will also have the opportunity to take part in leading on key disease areas (monitoring trends in treatment, staff training and prescribing) and assist in developing appropriate healthcare services, patient participation and health promotion. Involves working amongst a multidisciplinary team in the Estuary View UTC. Independently and collaboratively managing both booked and walk in patients appropriate to the skill set of the appointee.

We run various AQP contracts including Cataract Surgery, Ultrasound, Dermatology, Physical Therapies and Audiology together with 4 further community contracts, a Day Surgery Suite and an Urgent Treatment Centre with digital x-ray.

Job description

Job responsibilities

Clinical responsibilities:

In accordance with the practice/UTC timetable, as agreed, the post-holder will make him/herself available to undertake a variety of duties in a timely manner including: - - surgery and UTC consultations - managing emergencies - telephone and online consultations - triaging and visiting patients at home - checking and signing repeat prescriptions - request clinically appropriate tests and manage results referral letters (NHS and private),paperwork and correspondence - patient medical reports and examinations (e.g. Insurance Companies) on behalf of the Practice

Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation

Recording clear and contemporaneous computer based consultation notes to agreed standards

Collecting data for audit purposes

Compiling and issuing electronic acute and repeat prescriptions (avoiding hand-written prescriptions whenever possible)

Prescribing in accordance with locally agreed or national guidelines;

In general the post holder will be expected to undertake all the normal duties and responsibilities associated with a GP working within primary care and uphold GMC professional standards

Other responsibilities within the Practice:

Support other team members by providing clinical advice within scope of expertise

Promote a learning environment for patients, nurses and other health professionals.

Contribute to the planning and implementation of teaching for the multidisciplinary team and students associated with the practice

Compliance with all relevant National and Practice policies/guidelines available via intranet including but not limited to:- - Prescribing and QOF standards - Confidentiality - Safeguarding - Data protection - Health and safety - CQC quality standards

A commitment to lifelong learning and audit to ensure evidence based best practice;

Maintaining personal appraisal documentation and appraisal/revalidation requirements

Contributing to evaluation/audit and clinical standard setting within the organisation;

Contributing to the development of computer based patient records;

Attending training, Practice Meetings and events organised by the Practice or other agencies where appropriate;

Meet all tight timescales/deadlines for audits and written returns to ensure that the Practice meets quality standards and receives the designated funding (eg Enhanced Services returns, annual QOF audit etc).
