Job summary

Whitstable Medical Practice is looking to appoint either a qualified or soon to be qualified Advanced Clinical Practitioner to support adults living with frailty within Whitstable.

The post is for 37.5 hours per week, work pattern to be confirmed. We offer a competitive salary depending on experience and the NHS pension is offered.

Main duties of the job

The candidate musthave a special interest in frailty, have considerable post registration experience, be competent in assessing patients, arranging investigations, considering differential diagnoses and implementing management plans. It is essential to have professional registration.

The post holder will work alongside experienced Frailty Practitioners in addition to a number of other Allied Health Professionals. They will have strong organisation skills, be flexible and show empathy and compassion. Excellent communication and interpersonal skills with evidence of leadership qualities are also required.

About us

Whitstable Medical Practice is comprised of three sites and makes up is its own Single Practice Primary Care Network. The practice provides healthcare services to over 40,000 patients in the area. As is typical across the country, we are experiencing an increasing elderly population which is placing additional pressures on the local health economy. There are 10 care and nursing homes in the area all of whom are registered with the practice.

Job description

Job responsibilities

The post holder will apply clinical expertise, levels of judgement, discretion and decision making to the provision of clinical care of patients within the local community of Whitstable. As an ACP they will be expected to exercise a degree of autonomy through the application of clinical skills in relation to the assessment, diagnosis, treatment and the management of the patient.

They will regularly collaborate with the GPs at the Practice, consultant geriatrician and other community teams to meet the needs of the patients, upholding National, local and practice policy and procedures

The post holder will ensure that patients, their families and carers are supported and kept informed with the care and management of the patient.

  • Registered Practitioner holding current registration with NMC, HCPC or therapy practitioner
