We operate a total triage model and are very dynamic in our approach to attending to patients needs. We use EMIS, Docman and AskMyGP. We have a very strong social Media outreach presence.

This post is suitable for staff who have completed their Nursing Associate training and wishes to provide the highest standard of individualised and holistic patient care. The post holder will be expected at all times to work within the NMC Standards of Proficiency for Nursing Associates to meet with individual needs of the designated patient population. The post-holder must provide and maintain the highest possible standards of care for patients as well as provide assistance to the doctors and other members of the team. They will do this independently and with nurse and GP colleagues. They will also lead on cervical screening as well as immunisations
Main duties of the job
Job Responsibilities:
Mandatory (clinical):
All immunisations and vaccinations
Cervical smear screening
Primary care experience
Mandatory (non-clinical):
Maximise QoF and Enhanced Services achievement and attend any training or updates necessary.
Desirable (clinical):
Childhood Immunisations
Child Health Surveillance.
Contraception and sexual health advice and management.
Travel Advice & Vaccines
Routine Injections
Hypertension and blood pressure monitoring.
Health checks
Asthma and COPD monitoring and management.
Initiate, perform and interpret diagnostic procedures as appropriate for chronic disease management.
Desirable (non-clinical):
Development of recall procedures, appropriate literature and health promotion, and audit.
Identifying the risks involved in work activities and undertaking such activities in a way that manages those risks across clinical and patient process
Making effective use of training to update knowledge and skills, and initiate and manage the training of others across the full range of infection control and patient processes

To continue professional development to maintain the highest level of competency required for your role through annual updates and any training as required.
Always adhere to practice and professional guidance, policies and protocols in relation to your role and the care of patients.
To always have patient care as the highest priority, using your expertise and clinical skills to accurately assess patients individual needs and manage their treatment using practice protocols, PGDs and the appropriate guidelines.
Work with other healthcare professionals to monitor and manage patient conditions, care, monitoring and treatment.
Identify, manage, advise and support patients at risk of developing long term conditions, and deliver opportunistic health promotion.
Understand and apply policy that supports the identification of vulnerable children and adults, being aware of statutory procedure and local guidance.
Actively participate in QoF and Enhanced Services activities.
Actively participate in audit activity and policy and protocol development for the evaluation and development of surgerys care and service to patients.
Support other members the team at all times both clinical and non-clinical.
Take responsibility for their own work, as well as their learning and development at all times.
