Job summary
We are looking for a Practice Nurse to join our team at Whitby Health Partnership. We are seeking a Band 6 nurse who ideally has experience dealing with Asthma and COPD.
You already have experience in primary care, community nursing, or urgent treatment centre; or you may currently be working in secondary care in the management of long-term conditions and looking to make the move into General Practice. You will ideally have the following additional qualifications or equivalent experience:
Diploma in the management of a particular clinical speciality which have high demand amongst our patient demographic, e.g. Diabetes, Cardiovascular, Respiratory.
The role requires resilience and the ability to be flexible and adaptable. You will be confident to work alone without direct supervision at times, but you will also be part of a wider nursing team.
Main duties of the job
General Practice Nurses deliver care to the practice population, but also need to have an understanding of the public health profile and population needs in order to be proactive in ensuring services are as far as possible, matched to need.
About us
Whitby Health Partnership is a large GP Surgery with 16,200 patients offering a variety of health services to its community. We are a new partnership with ambitious plans for the future development of the practice.
The current structure of our nursing team consists of a GP Lead, 4 practice nurses and 2 healthcare assistants.

Develop, implement and embed health promotion and well-being programmesImplement and evaluate individual treatment plans for chronic disease patientsIdentify, manage and support patients as risk of developing long-term conditions, preventing adverse effects on the patients healthProvide routine nursing care to patients as required in accordance with clinical based evidence, and local and national guidanceProvide travel medicine servicesRequest & process pathology services as necessaryProvide chronic disease clinics, delivering patient care as necessary, referring patients to secondary / specialist care as requiredMaintain accurate clinical recordsMaintain chronic disease registersPrioritise health issues and intervene appropriatelySupport the team in dealing with clinical emergenciesRecognise, assess and refer patients presenting with mental health needsSupport patients in the use of their prescribed medicines or over the counter medicines (within own scope of practice)Liaise with external services / agencies to ensure the patient is supported appropriately (vulnerable patients etc.)Delegate clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual)Support the clinical team with all safeguarding matters, in accordance with local and national policiesDeliver opportunistic health promotion where appropriateIf you are keen to join a superb Practice Team who enjoys celebrating success and being genuinely supported in your own development and the development of others then please apply.
