Job summary

New River Health is a practice of 19,000 patients across 2 sites in Ware (Church Street Surgery) and Hertford (Castlegate Surgery), Hertfordshire. We are a training practice.

Main duties of the job

The Practice-based Clinical Pharmacist will improve the quality and safety of care offered to patients of New River Health. They will have regular contact with patients and will reduce the workload of GP prescribers in the practice by performing structured medication reviews. They will also support the management of patients with long-term conditions by reviewing repeat medication requests.

They will co-ordinate prescribers at the practice to ensure that appropriate medication is prescribed and maintained whilst fulfilling the requirements of ICB and locality initiatives.

Qualifications

Essential

  • Masters degree in Pharmacy (MPharm) or equivalent
  • Registered with GPhC
  • Member of RPS

Desirable

  • Postgraduate knowledge through a Postgraduate Diploma and qualifications (e.g. clinical, community, therapeutics) or equivalent.
  • Independent prescriber status
  • Minor illness practitioner
