We are looking to recruit an experienced Practice Nurse to join our clinical team at our friendly practice in Blackheath SE London. The Blackheath Standard Surgery is a busy training practice with a supportive team and a commitment to providing caring, quality healthcare for all our patients. Practice Nurse experience is essential.

Main duties of the job
The sucessful candidate should have good general practice experience in managing long term conditions including Asthma/COPD, Diabetes,Hypertension, Child Immunisations, Cytology and Travel Vaccinations.

We have an exciting opportunity for a dedicated, experienced nurse to join our team comprising os two GP Partners, two salaried GP's, one Practice Nurse two HCA's and an experienced and supportive reception/Admin team. We provide a high level of care to approx. 6,800 patients.

be responsible for the implementation of processes for the effective management
of patients with long-term conditions using evidence-based practice including
care for elderly and housebound patients.
Working
as a member of the practice multidisciplinary team, the post holder will ensure
nursing services are delivered effectively to the entitled patient population.
The
Practice Nurse will be responsible for a number of clinical areas such as infection prevention and control, health promotion, chronic disease management, health
promotion, as well as actively supporting the practice management team in the
reviewing and delivery of clinical policy and procedure.
