Scope
We
have an exciting opportunity for a Pharmacy Technician to join our
multi-professional Primary Care Team working with our Clinical Pharmacists.
The
purpose of the role will be to lead improvements to ensure the delivery of
safe, effective and efficient systems for repeat prescribing, medicines
optimisation, reducing medicines waste and maximising patient outcomes. To
promote repeat dispensing, online ordering and help patients to get the best
from their medicines, including switching when appropriate.
Main duties of the job
Key
Roles and Responsibilities
To
respond to medication queries from patients & staff, liaising with appropriate
clinicians when needed.
Undertake
patient supporting roles to ensure effective medicines use, through shared
decision-making conversations with patients.
Carry
out medicines optimisation tasks including effective medicine administration (e.g.
inhaler technique), supporting medication reviews & medicines reconciliation.
Where
required, utilise consultation skills to work in partnership with patients to
ensure they use their medicines effectively & to reduce medicine wastages.
To
support the pharmacists in providing Structured Medication Reviews (SMRs) e.g
organising necessary monitoring tests.
Support
medicines reconciliation for new patients (incl. care homes) &
synchronising medicines for patient transfers between care settings & linking
with local community pharmacists.
To
actively manage discharge summaries and clinic letters, actioning and updating
patients records, reconciling medications & liaising with patients when
necessary.
Manage
shared care protocols and liaise with Clinical Pharmacists for more complex
patients.
Provide
expertise to address both public health & social care needs of patients,
including lifestyle advice, service information, and help in tackling
inequalities.
Support
initiatives for antimicrobial stewardship to reduce inappropriate antibiotic
prescribing.

We
are a two-site dispensing practice on the edge of Exeter extending onto the countryside
of East Devon with an expanding population, currently at 17,000 patients. The
area boasts access to city life, Dartmoor and beautiful beaches.
We
are a welcoming, friendly team, who pride ourselves on our supportive
environment. The successful candidate will be joining our team of GPs, Practice
& District nurses, Occupational Therapist, Dietitian, Paramedic and two
Clinical Pharmacists.
You
will receive clinical supervision and be supported by our Clinical Pharmacists
and work within the PCN pharmacy network, attending meetings and developing
network opportunities. An essential part of this role will be to complete the
PCPEP (Primary Care Pharmacy Educational Programme), a 15-month development
programme run for new PCN pharmacy technicians by CPPE (Centre for Pharmacy
Post Graduate Education).
The
core role responsibilities will cover clinical, technical and administrative
areas.

Essential
- Professional registration with GPhC
- BTEC/NVQ level 3 or equivalent in pharmaceutical services.
- To enrol on and complete the PCPEP (Primary Care Pharmacy Education Programme) when in post.

Experience
Essential
- Experience of working as qualified, registered pharmacy technician in primary care, community or hospital pharmacy for at least 3 years.
Knowledge and Understanding

- Knowledge and understanding of pharmacy law and ethics and current legislation.
- Good clinical pharmacy knowledge including terminology
- Sound computer skills, proficient with MS Office.
- An excellent understanding of data protection and confidentiality issues.
Desirable
